Baby Can I Hold You is the fourth episode of the twentieth season and the 424th overall episode of Grey's Anatomy.

In the morning, Amelia is looking for her charger among Lucas's stuff in her living room. He drank the last of the coffee. He rushes out for an important meeting.
Bailey runs into Ben in the lobby. Link cleared him to return to work. Ben is happy to be fully back but he assures Bailey that he'll be careful. She fakes enthusiasm as he leaves.
The interns are meeting with lawyers. Benson is asked if he knew about Maxine's DNI when he intubated her. He tries to talk himself out of it. Jules wonders why this interview is happening as Maxine is not suing. It turns out her son launched an inquiry into it. Mika is asked about leaving Sam to tend to Teddy. Lucas says Sam was dying within a matter of seconds. The lawyer questions his expertise. Lucas claims he knows the vitals of a dying patient. Simone's testimony backs him up. She is asked if she would have opened up the patient if Lucas hadn't told her to.
Amelia runs into Owen as they wait for the elevator. She vents about Lucas not picking up after himself. Monica arrives, too. Amelia decides to takes the stairs.
The interns tail Bailey, who tells them lawyers are paid to make you feel bad. She urges the interns to focus on the work. Mika is one chest tube away from completing her log. They arrive in the lecture hall where they meet Arizona Robbins.
Arizona presents the case of Vida Madera, who is 30 weeks pregnant and has a history of vision loss. Jo and Bailey met her through the clinic. An ultrasound revealed a vein of Galen malformation. It's AV malformation causing dysregulated bloodflow in the affected area of the brain. Usually, the mother and placenta's circulation protect the baby but once the cord is clamped, the baby's heart and lungs are overwhelmed, potentially causing heart failure, seizures or death. Standard procedure is embolisation after delivery but many babies do not survive, or with major brain injury if they do. That's why Bailey reached out to Arizona. She started a clinical trial on fetal brain surgery to fix the defect before birth. The procedure has never been done before, which surprises Bailey. They're making history today.
Teddy meets Winston and Richard in front of the OR board. She has decided to return to work since they still have not hired a new attending. She has been practising in the skills lab. Winston openly doubts her stamina at this point.
Vida's surgical team and the interns enter her room. Vida and her husband read up on Arizona's work. Simone goes to present the case but Vida interrupts her as she has changed her mind. She can't have the surgery. She has been through too much and has decided to take the very small chance that the baby will make it without major neurological deficits. Bailey has the interns clear the room.
Down in the ER, a man asks Mika about Dr. Millin, whom he had paged. She's sure Millin will be with him soon. She informs Owen that she's one chest tube away from completing her log. He directs her to a metal disimpaction. Jules arrives so Mika directs her to the man, whom Jules does not recognize. This was not the Dr. Millin he meant. A man in scrubs then appears, who turns out to be Jules's brother Doug. Jules says he is not a doctor.
Outside the room, Bailey informs Jo and Arizona that Vida wants to be discharged. Bailey does not appreciate that Arizona withheld the fact that Vida was the first patient in the trial. Bailey says it's over now. Jo says it's a statistical miracle that Vida's still a viable candidate for the trial. There's little time for Vida to change her mind. Arizona's hands are tied. It's over.
Bailey meets Winston, Levi, and Benson in Dorian's ICU room. Dorian just woke up but he's disoriented and combative. Winston tells Dorian where he is, which calms him down a little. Dorian remembers two guys and maybe a gun. Levi informs him that his parents know that he's here. Benson confirms to Dorian that he was shot and briefly talks him through the surgeries they performed on him. Winston leaves for the OR while Bailey instructs Benson to run an infectious work-up and fix Dorian's arterial line.
Teddy meets Link and Richard in the skills lab. They're hosting a mock OR. The patient (doll) is a man who came in for a CABG. Richard simulates a cardiac arrest. Teddy decides to play along and starts CPR while ordering epi.
Lucas finds Simone and asks about the lawyers. She dodges the question and leaves to check on Bailey's post-ops. Amelia then comes to scold Lucas for borrowing her car without asking. Lucas is on her service and leaves to round on her patients. Monica overheard them. Amelia says the fatal surgery got cancelled. Monica understands it's terrifying for the parents.
Jules is not amused with her brother invading her workplace. Doug says that Joshy is one of his subscribers. Doug works as a wellness influencer after going on one yoga retreat, which does not make him a doctor. Doug claims it's just branding. Joshy is in a lot of pain. It started as an ingrown hair but it's started to swell up. It looks like an infected abscess, which grosses Doug out.
Mr. Videra chases Arizona down as she's on her way out. Arizona says she can't do the surgery without consent or their trust. Mr. Videra says she has hers and he thinks she can get Vida's too. She just needs some time. Vida won't survive if the baby doesn't. Mr. Videra begs Arizona not to leave.
Jules asks Mika to help her drain an abscess. She made an incision but nothing's coming out. She'd rather not call an attending because that would mean they'd meet her brother, who pretends to be a doctor. Mika agrees to help out if she gets to ask Doug all the questions she wants about Jules's childhood.
Benson briefs Levi that Dorian's subclavian line culture came back positive. Levi instructs him to remove it and place a new one on the other side. Benson has done two before under supervision. Levi tells him to do it now on his own.
Arizona has gathered the team in the skills lab. She explains they'll have 20 minutes after injecting the paralytic. Vida will be awake the entire time, Bailey reminds the interns. Amelia is practicing on 3-D-printed phantom skulls. Simone and Lucas question them practising without having convinced Vida. Amelia says they don't convince people and throws the interns out, but Arizona stops them. She asks if anyone's good with a 3-D printer.
Doug tells Mika Jules was already quite mean as a kid. He then reminds Joshy to properly hydrate after the procedure since the body's 60% water. Mika makes another incision and applies pressure, yet no pus comes out. Mika decides it's time to call Owen. Doug grows queasy upon seeing the abscess. Jules instructs him to wait outside and not embarrass her or talk to anyone. If he does, she will hire a bot farm to drag him on every message board on the Internet.
As they wait for the 3-D printer, Lucas tells Simone that Amelia was annoyed at him, not her. Simone confesses she told the lawyers she would not have opened up Sam without Lucas present, which she says is the truth. He's taken aback by the fact that she threw him under the bus. She points out they didn't get to align the stories since he walked out. He states he has to focus on work and leaves.
Benson is inserting the subclavian line. Dorian asks how many times he was shot. Benson tells him three times and says they'll need to evaluate his nutritional status given the amount of bowel that had to be removed. Dorian has trouble breathing all of a sudden. Benson has Levi paged.
Arizona returns to Vida's room. She talks about Dr. Herman, her colleague, having low vision and how she likes Arizona to walk her through the surgeries. She will do the same for Vida. She hands Vida a 3-D printed model of the baby's face, based on this morning's echo. She then shows her the MRI and points out where they'll access the skull and where the malformation is. Arizona knows it's hard not being able to hope based on previous experiences. Arizona stresses that she knows exactly what she's doing. She knows it's painful but this pain has purpose, it could have life on the other side of it.
Levi arrives in Dorian's room. Dorian's X-ray reveals that he dropped a lung. Dorian needs a chest tube. Levi tells him not to wait around and do it. Benson starts doing so as Levi watches him. Benson freezes and has Levi take over.
Owen meets Mika and Jules in the procedure room. Based on the ultrasound, he'd say the abscess is located and deeper. He instructs Jules to insert her finger and palpate for the right pocket. She then makes the deeper incision, causing bloody pus to fly out. Joshy feels better. Doug collapses outside.
Teddy scoffs at the poor quality of the model. She's good to continue. This phase will test her fine motor skills by completing an anastomosis. Link struggles as a scrub nurse. Winston comes in to check and Teddy assures him that her stamina is okay. Winston still won't clear her. Recovery for open heart surgery is a month minimum, and she might not be fine tomorrow. Richard says he'll take Teddy to the OR and sign off on her return if she does well. Winston agrees to sign off if Richard does.
Amelia finds Monica giving instructions to Lucas. Since the surgery was cancelled, she figured she could use him on her post-ops. Amelia tells him to prep Vida. He runs into Arizona and informs her that she won't be seeing him in the OR as interns are not allowed there.
Arizona drags Bailey into a supply closet and asks her what happened to her. She remembers Bailey's trying to cure a kid with SCID. Bailey received an award for teaching and Arizona was really excited to stand by her and teach today, and now she's found out interns are not allowed in the OR. Today's surgery is magic and if Bailey won't let the interns see it, how will they be motivated to get back in the OR?
As they get gowned and gloved, Teddy gushes to Richard about feeling excited about doing this VATS. It reminds her of her first procedure. As Teddy takes her place by the table, Richard leaves without a word.
Arizona and Amelia enter the OR, where Jo has just administered the spinal. Arizona once again asks Vida's consent. She's ready. Bailey then comes in with the interns and instructs them to stand against the wall.
Jo struggles to get the baby in the right position. Benson is paged away by Levi. Jules's phone rings. It's Doug. She leaves, much to Bailey's delay. Jo then gets the baby in the right position. Arizona injects the paralytic and anaesthetic into the baby's thigh. Once the baby is still, Amelia is handed the needle and Arizona reminds her that every second counts.
Jules meets Doug in the ER. He had her paged since he woke up alone. She tells him she was in surgery because she is an actual doctor after spending every dollar and every ounce of energy it took to earn that title. She's done with anyone who tries to take that from her. Owen overhears her yelling and steps in. He tells Doug that Jules can't treat him as his family, so he'll say it instead. Misleading others on their health is questionable at best and can have severe consequences, so Owen recommends other ways to influence people. By the way, he's related to an excellent physician. He's free to go now.
With 14 minutes on the clock, Amelia says they need about three more coils. Arizona suddenly stops Amelia. She calmly says the baby is moving slightly so she needs to administer more paralytics. She quickly pulls it off. Amelia realizes the need to move fast.
Teddy finds Richard in the OR gallery. Beckman's doing the VATS. Richard has been away from the OR for about three weeks and he figured today was the day to return. Upon stepping back in, he did not feel right. Teddy assures him it's still there. She also watched him today. He always had her back and she will do the same for him. If she ever feels like he does not belong in the OR anymore, she will tell him.
Down in the OR, Amelia has deployed all coils. She removes the needle. Arizona compares the pre-op ultrasound with the current one. The malformation is repaired. Bailey agrees that it's magic. She turns to the leftover interns, who are daydreaming or on their phone. Bailey rolls her eyes.
Benson has updated Dorian's parents. He tells Levi that he saw Maxine in his head when Dorian was struggling to breathe. Levi understands that the meeting with the lawyers spooked him. Levi says the memorable complications follow you, whether the outcome was good or bad. It'll dull over time.
Arizona happily informs Vida and her husband that the procedure went well. The baby's cardiac output has decreased and it looks promising. They'll follow up closely with imaging. After birth, the baby will need to spend a month in the NICU and monitored for the first two years of its life. Vida thanks Bailey for the clinic and introducing them to Dr. Robbins.
Jules runs into Mika at the vending machine. Mika found Doug to be spoiled and infuriating. Jules had to bail him out of jail so many times, always for dumb stuff. In her family, she was always the one they'd call if something went wrong. Mika's mother always told her it was the big sister's job to watch after the younger kids. Jules says Maxine is more family to her than anyone in her actual family. Maxine's son doesn't want her to be Maxine's medical proxy anymore. Mika says without their mistakes, Maxine would still be on a vent or dead. Mika finally gets the recognition from Jules that she didn't do anything wrong that night.
Amelia arrives at a nurses' station looking for her keys, which Lucas texted her are “at the desk”. They're not there. Amelia has Lucas paged for a word as Monica passes by. Monica thinks she really has it out for that intern. Amelia explains he's her sloppy nephew who has her car keys. Monica offers her a ride home, which Amelia declines. Monica thinks she should cut him some slack since his aunt performed groundbreaking surgery while he can't enter labs correctly. Monica congratulates Amelia on her surgery.
Arizona meets Bailey in the lounge. They made history today. Bailey explains why she sidelined the interns and how they're almost done with their procedure logs. Arizona thinks going back to the basics won't stop the tiny surgeons from making mistakes. She knows Bailey knows that so she wonders what is truly going on. Bailey says Ben broke his arm on duty but today, the brace came off and he's back to the dangerous part of the job. Arizona tells her she has to trust that it'll be okay. Bailey says it's not easy for her. Since the interns didn't appreciate the magic, she still won't let the interns into the OR. Arizona says that is the Bailey she knows and hugs her.
Mika informs the other interns that she completed her log. She works hard and she's a good doctor, unlike them, who gravitate towards malpractice. Simone asks Lucas who's rounding on Vida in the morning. “Whatever,” he says. She urges him to at least be civil. Bailey enters the locker room. She gave them the opportunity to watch medical history being made, but they only care about procedures or who's at fault. Everything in that OR happened because some of the best surgeons put their heads together. They need to learn to work together, so she has a new rule. Nobody gets to operate until everyone has completed their log. Mika is disappointed.
Lucas comes home at Amelia's apartment. He bought more coffee. She knows intern year is hard. She nearly flung out the first few months, too, and she did alienate her friends when she wasn't sober. He asks if she ever had to live in her family's shadow. She says Derek was a legend. He has no idea what it was like to work at Grey Sloan with him. Lucas was talking about her. She invites him to sit down and watch the movie with her. Lucas wonders what if he's not cut out for this. She thinks he'll be fine, and if not, the family legacy will die with her.

Vida, 26, was 28 weeks pregnant when her fetus was diagnosed with a vein of Galen malformation. Two weeks later, she was in a clinical trial to have Arizona Robbins operate on her fetus prior to delivery. Vida had initially agreed, but then said she couldn't have the surgery. Wanting to give her baby the best chance they could, Arizona 3D printed the ultrasound and explained the surgery one step at a time. Vida agreed to have the surgery. There was a slight complication when the fetus started moving, but Arizona was able to inject more sedation and the embolization continued. Vida was told that the surgery went well and they would follow the rest of her pregnancy and after the baby's birth.
