Hospitals have a long list of unspoken rules. Never say, "It's quiet," unless you want to be blamed when all the pagers go off. If your attending is standing, you better be, too. And if you finish the coffee in the lounge, it's on you to refill it. No one tells you these rules, but you break them at your peril. Rules, written and unwritten, help keep the trains on the tracks. Without them, hospitals would devolve into chaos. But following all those rules can be stifling. There's no room to be yourself, to chart your own path. Sometimes you have to break the rules and go with your gut. I promise it'll only hurt a little.

Bust Your Windows is the fifteenth episode of the twenty-first season and the 445th overall episode of Grey's Anatomy.

Short Summary[]

A window washer's platform crashes through a hospital window, testing the Grey Sloan team's limits. Meanwhile, Blue and Simone clash over their living arrangements, and Lucas strives to impress Catherine while on her service.

Full Summary[]

Simone is walking down the hallway when she sees Oliver in Lucas's old room. He says he's Blue's med school roommate's cousin's boyfriend.

Ben gets frustrated while studying. Bailey comes in and notices the wind has picked up. She asks Ben to take down the patio umbrella, but he already did. He wants to have a date night. She wants to watch a movie, but he wants her to quiz him. She says that's a job for a co-resident. He needs to get to know them so they'll study with him.

Richard asks Winston if he likes dogs. A couple of pugs moved in next door and he's not a dog person, but he's taken a liking to them. Their owner is recently-divorced. Winston knows he's trying to set them up and isn't interested. As Winston walks away, Lucas brings Richard a thank you present. Richard hasn't talked to Catherine yet. He tried, but he thought it would do more harm than good. Lucas understands and thanks him for trying. Richard does have an idea. Bailey comes by and says Lucas is in the clinic today. He goes with her.

Simone asks Benson who Oliver is and gets the same complicated explanation. He's interesting at Seattle Pres, so Benson offered him a place to stay. He'll be out in a few days. He asks if he should let Simone know if he rents the room out. She's upset that he's looking for roommates without her. He says it's short-term and it'll pay for their utilities. Simone was thinking of asking Lucas to move back in. It's her house, too.

Lucas tells Bailey he tried to drain an abscess, but one of her OB/GYN trainees beat him to it. He asks if she's sure she needs him. Bailey says she didn't ask for him. It was Richard's doing. Lucas is confused, but then Catherine comes in. She's late because she had to yell at the president. Bailey sends her to a patient in bed three with recurrent abdominal pain. Lucas is surprised to see Catherine there, but she views it as a way of tithing since she can't always get to church. Lucas offers to be her intern for the day and she accepts.

Simone is venting to Jules, wondering who brings people into your house without asking you. She reminds Simone she invited Benson to live there without asking Mika. Simone reminds her that she used to sleep with Benson. Jules says things must be going well if she's planning to ask him to move back in. She's happy for Simone even though her relationship ended. The kids around them start to exclaim and so does Gregory James as Beau Caraway appears in the window, wearing a cape and ready to wash the windows. Beau waves to the kids.

Ben makes small talk with Taryn, but she doesn't have time for him.

Simone presents Yusuf Farid, who is two days post-op. In the bed, Yusuf fights with Safi Farid, his brother. Their mother, Elham, tells them to turn it off and listen to the doctors. Her husband is traveling for work, so she's on her own. Simone examines Yusuf and says he has no bowel sounds. Richard says they have to make sure his intestines have woken up before he can go home. Passing gas is a good sign. Safi says he's an expert at farting and they start fighting again. Elham sends Safi out to the waiting area. Yusuf wants to go home and see his dog, but he has to stay until he passes gas. Simone tells him there's a superhero climbing the walls of the hospital. He only comes twice a year, so Yusuf is lucky.

Catherine and Lucas go to Navy Sutherland, who is in the clinic with her friend, Bella. Navy has abdominal pain, nausea, and vomiting. Lucas goes over her labs and says he suspects a UTI and kidney infection. Catherine asks him to back up a little. She introduces herself to Navy. Navy is staying with Bella while her parents are on a cruise and she threw up in Bella's bed. Navy's mom yelled at her to get to the clinic. Catherine examines her and Navy celebrates getting a text from Caleb Scott asking her to tutor him in algebra. She thinks he likes her. Bella says he might just suck at algebra. As Catherine presses her abdomen, Navy winces in pain. Lucas offers to order an MRI, but Catherine wants to finish the exam first. She sends him to find an ultrasound while she asks Navy about Caleb.

Richard asks Winston for some consults. Owen and Teddy are on vacation, which is inconvenient for him. Jules asks him to sign off on a patient discharge, but she's distracted by the wind blowing the window washer's cart around. He swings around and then crashes through the window.

Jules, Winston, and Richard rush to help Beau, who is trapped on his rig and has pain in his neck. Winston has Jules stabilize his neck as Safi comes over and tells Richard something is wrong with his mom. Richard steps away to check on Elham, who was hit with a chair. She feels like the wind was knocked out of her. They need to get her downstairs. Elham tells Safi to keep an eye on Yusuf and try not to argue.

Navy is trying to decipher what Caleb's messages mean and Catherine says people used to be more direct in her day. If you wanted to talk to someone, you just climbed up a tree and knocked on their window. Lucas asks Navy to pull her shirt up, but she doesn't want him doing her ultrasound. Catherine pulls him aside and explains why Navy is having trouble. She sends him away.

Benson, Ben, and Taryn arrive in the ER to help with the injured. Benson starts Elham's IV as Simone finds free fluid in her abdomen. They need to do a CT scan. Taryn, Ben, and Benson do rock, paper, scissors do decide who gets to go with her. Benson wins. Elham asks someone to check on her sons. Richard tells Ben and Taryn about Yusuf's condition and sends them to get him to walk.

Navy's ultrasound shows blood in her uterus. Bailey asks when her last menstrual cycle was and Lucas says she hasn't had one yet. He wonders if it's an imperforate hymen. Catherine gets a phone call about the broken window. She has to deal with it and hands Navy off to Bailey. Lucas goes to follow Catherine, but she says she doesn't need help calling the insurance company. Lucas just wants to impress Catherine, but she won't let him near the patient and now she's gone. Bailey asks if he really thinks he can get her to change her him when Bailey couldn't.

The firefighters arrive and start extracting him. Jules puts a vitals monitor on him and says his emergency contact will be there soon. He hopes it's not too soon because she'll freak out. He knew it was windy, but he didn't want to disappoint the kids. They're ready to move him as soon as the power is cut. Beau's father was also a window washer. He didn't want Beau to be one, but Beau was miserable as an accountant. He loves being a window washer. Beau suddenly crashes. Winston has to intubate him even though he's upright.

When Simone comments on Benson starting the CT before she has said to, he says that his friend texted him at 1 AM when Oliver's rental fell through. He's sorry he forgot to text Simone. Richard comes in as Elham's scan comes up. She has free fluid in her abdomen, but no obvious organ damage. Simone thinks they should do a diagnostic laparoscopy. Richard agrees and tells them to book and OR.

They're rushing to stabilize Beau as Leah Caraway comes in. A nurse holds her back, but she introduces herself and says they called her. Winston says they're working on her son. She corrects that he's her husband. The power is off, so they're finally able to cut Beau out. As soon as they cut the cable, he starts bleeding from his neck. Jules holds pressure and they prepare to move him as Leah tells him she loves him.

Lucas asks if he can go to the ER since she doesn't need him. She says she does need him and has him come into Navy's room, where she tells Navy she has a transverse vaginal septum, which means the blood from her menstrual cycle hasn't been able to pass through her vagina. They can fix it, but they have to consult with an OB/GYN and talk to her parents before they make firm plans. Bailey has Lucas get that rolling.

Ben tries to bond with Taryn again, but she's trying to read Yusuf's chart. Every kid tries to get out of walking. The only difference is how they do it. Taryn and Ben rock, paper, scissors to decide who will go with Yusuf and who will talk to another patient's parents. Taryn loses and looks into Yusuf's room, where he and Safi are fighting.

Catherine comes back and asks about Navy. Bailey hoped there was a new treatment, but all they have is medieval nonsense. Lucas says they'll place a drain into the uterus to collect the blood while she takes contraceptives to stop her cycle and then they'll fix her septum. She'll have a bag of blood outside her body for weeks. Catherine says she isn't equipped to handle that. Lucas says they'll find another way, but Bailey says the OB attending couldn't find one. Lucas thinks there has to be something out there. Catherine challenges him to find it. She knows he's trying to impress her, so he needs to do something impressive.

Beau has significant tissue loss in his carotid. Jules thinks need to place a shunt to secure blood flow to his brain, but Winston says his other carotid is intact, so they're going straight to repair. It's riskier, but more efficient.

Bella asks Lucas how much longer it'll take. He asks if she has somewhere to be and then he sees that Caleb is calling her. She says Navy didn't even like him until he asked her to tutor him. He asked if Bella wanted to go see a movie with him. In twenty years, Navy won't even remember his name. Lucas says she'll remember Bella's and how she treated her. Bella says he can't predict the future and leaves. Simone comes in and starts venting about Benson. He says Mika was their buffer. Simone continues her ranting and Lucas says he's trying to find something that doesn't exist. He asks if it'll be weird next year when he's an intern and she isn't. His plan to get her to reconsider isn't going well and he only has a month left. She thinks maybe it won't be that different. He says he needs to get back to work and she leaves.

Taryn goes to Ben to ask for help. Yusuf is refusing to walk. He started crying as soon as he heard his mom needed surgery. Ben grabs a stuffed penguin and says he probably takes bribes.

Yusuf is worried about his mom. Taryn says he still has no bowel sounds. Ben thinks a walk could take his mind off things, but he doesn't want to walk. Ben says it'll make him feel better. Safi says he's waiting for the superhero. Taryn starts to say he's not coming today, but Yusuf won't leave his room until he sees the superhero.

Lucas hands Bailey and Catherine papers. He found case reports in Australia using suction and irrigation to remove the blood with minimally scarring. They look it over and say they think it'll work. Since Lucas found it, Bailey says he can do the procedure. Bailey asks if Catherine will consider keeping him with his class if he does well. Catherine is irritated with this. Lucas says he's doing well in every area. He knows he made a mistake with Sam Sutton and he'll think about Sam every day for the rest of his life. Catherine says they'll talk after she sees how he does. Lucas thanks Bailey for her help.

Gregory comes in to get an update for Beau's wife. Winston says his surgery is going well. Jules teases Winston for thinking his wife was his mother. Winston says there's a difference in their ages. Jules says Maxine dated a 100 year old when she was 82. They didn't care until he died. The heart wants what it wants. Winston is ready to check the blood flow. He asks for doppler. There's no blood flow, so there must be a clot. There's no time to page neuro, so he has to remove it himself. As Jules starts to panic, Winston removes the clot. Blood flow has been restored.

Simone and Benson navigate Elham's abdomen as Richard observes. Since they're bickering, when Richard spots a tear in the mesentery, he sends Benson out, saying he can't concentrate with whatever is going on between them.

Leah sees Winston and asks how Beau is. Winston says he's stable and Leah is relieved. Winston warns that they need to evaluate his neuro status, but they can't do that until he wakes up. Leah says she spent years watching her friends meet their partners and gave up. Then she met Beau. He pursued her and wouldn't take no for an answer. Then she decided to go for it. He makes her happy and there's not enough in the world that does. Winston understands. He apologizes for mistaking her for his mother earlier. She's gotten used to it. Winston promises to do whatever they can to help Beau. He also tells her Beau's parents called and will be there soon. Leah decides to go to the cafeteria because his mother isn't a fan of her.

Benson is up that he got kicked out of the OR because of Simone, but Simone says it's not her fault. He needs the OR hours because the days he took off with Molly left him behind. Simone asks if he knows what it's like to be a woman and see a stranger in your house. Benson says he's a good guy, but Simone doesn't care. He treats their home like a bachelor pad. Benson says she's always making out with Lucas everywhere. They both get paged because Elham is awake.

Safi has told Yusuf there are superheroes at the hospital to get him out of his bed. Yusuf says their mom will be mad is he's lying. Ben and Taryn, using hospital materials to dress as superheroes, appear in the hallway. Ben says they need his help to protect the hospital from the evil robots, really the computers. They're everywhere and he can freeze them by tagging them with the bandages of bravery. But to destroy them, he has to fart and engulf them in gas. He accepts the mission. Taryn ties a gown around him as a cape and he starts working.

Navy struggles to text Caleb. Lucas understands. Meanwhile, Bella laughs as she gets a text and Lucas says Caleb might not be the one for her if she doesn't feel comfortable texting him. Navy says every body makes her nervous. She's never done anything. She realizes what she's saying, but he tells her he's good at girl talk because he has four sisters. She's ready for her procedure and he says he'll see her there. She thought Bailey was doing her surgery. She's nervous about him doing it. He realizes this and pretends to get paged for a craniectomy. He says she'll be in good hands with Bailey.

Jules checks on Beau and tells Winston he has no sign of stroke. Jules says she wasn't trying to second-guess him in the OR, but she'd never seen that before. It's a good thing he has. She says he likes to pretend he never takes risks, but he just walked a tightrope without a net. He says he would never do that because he doesn't like heights. He says it's different with surgery. He has built up confidence after so many years. She'll get there. She asks him to come with her and promises they won't leave the building.

Richard explains Elham's condition to her. She'll able to go home in a few days. Benson brings in Safi, who is happy to see his mother. She's glad he and Yusuf didn't kill each other. Safi says Yusuf is trying to fart on some robots. Safi is sorry they fought. He promises to be nicer. Elham says siblings fight. She and their uncle used to fight all the time until they realized it was a waste of time. As they talk, Simone and Benson look at each other.

Yusuf is still hunting robots. He doesn't feel the need to fart. Taryn gets paged away, leaving Ben with Yusuf. Yusuf laughs and farts. He defeated the robots and Ben leads him back to his bed.

Jules brings Winston to the roof. She asks the last time he was there and he says Monday, but that was just to get a patient who'd been brought in. He didn't look out. She says there's a guy trying to steal a woman's purse. Winston comes to the edge and looks down, but finds Jules was lying. He tries to leave. She tells him he doesn't have to look down, but encourages him to look out. She asks if he knows what a carillon is. It's a set of bells you play like a keyboard. She used to play them in college. The bell tower became her favorite place on campus because it made everything else seem small. When Jules says she's cold, Winston gives her his jacket. They look for a few more seconds until they get paged. Beau has been extubated.

As they exit the elevator, Jules gives Winston his jacket back. Richard sees them and asks if they took a field trip. Jules says they just went to the roof to check out the view. Richard says it's lovely, but you need to be careful because anything could happen up there. Winston says they're going to go check on Beau.

Simone says she did Elham's post-op check. Benson thanks her and apologizes about Oliver. He'll make sure to tell her next time. She owes him an apology, too. She didn't realize it was hard for him to see her and Lucas. He says a lot has been hard, but he got over her before and can do it again. He says Lucas can move back in. Simone asks if he's sure and he says yes, as long as he pays for utilities.

Bailey tells Navy she did great. They got all the blood out and opened her septum. They're waiting for a bed to transfer her to the hospital. Lucas says he called her parents and they're on their way home. He asks where Bella is and Navy says she went to see a movie with her sister. Catherine comes in and asks how the surgery went. Navy thought he had an emergency skull surgery. He says he did and Bailey did her surgery without him. He leaves under the guise of checking on that patient.

Catherine follows Lucas out and he says he won't bother her anymore. She says it's too late for that. He's been a thorn in her side for months. What he did for Navy, giving up the procedure for her benefit, requires sensitivity and good judgment, which is where she thought he had a deficit. She tells him he can stay with his class. He thanks her and says she won't regret it.

Ben finds Taryn in the supply closet. Her possible appy was just gas. Ben says speaking of that, Yusuf passed gas. Ben asks if he did something to make Taryn dislike him. They're co-workers and he's been trying to be friendly. She says she doesn't have time. Without Levi, she's doing it all herself. They were studying for boards together and now she has no one. Ben offers to study with her.

Lucas tells Simone he doesn't have to repeat. He's excited. Now they'll finish their residencies together and can get a couples match for fellowships. He asks how she feels about New York. His family is there. They can also come back to Seattle to see her grandma. She says they still have to finish residency first. He knows, but he likes having options. He remembers she wanted to ask him something and she asks if he wants to go to Joe's for dinner. He says yes and they walk away together.

Medical Notes[]

Navy Sutherland[]

  • Diagnosis:
    • Transverse vaginal septum
  • Treatment:
    • Surgery

Navy, 14, came to the clinic with recurrent abdominal pain, nausea, and vomiting. Labs showed iron-deficiency anemia. Lucas suspected a UTI with pyelonephritis, but Catherine wanted to exam her first. An ultrasound showed that her uterus was filled with blood. She reported not having begun menstruating, so Lucas suspected an imperforate hymen, but Bailey diagnosed a transverse vaginal septum. She had Lucas page OB/GYN and get an MRI. Protocol was to place a drain to allow the blood to drain out over a few weeks and then repair the defect, but Lucas said there had to be a better way. He found case reports from Australia where they used an incision in the posterior wall of the vagina and suction to remove the blood, negating the need for a drain. Bailey said Lucas could do the surgery, but when he saw that Navy was uncomfortable with the idea, he backed out and let Bailey do it.

Yusuf Farid[]

  • Diagnosis:
    • Meckel's diverticulum
  • Treatment:
    • Meckel's diverticulum resection
    • Bowel resection

Yusuf, 9, was two days post-op on a Meckel's diverticulum resection and small-bowel anastomosis. He had no bowel sounds, so he had to stay in the hospital until he'd passed gas. When he found out his mom was injured, he refused to walk. Ben and Taryn dressed as superheroes and made a game to get Yusuf to walk around the floor. This worked and he was finally able to pass gas.

Beau Caraway[]

  • Diagnosis:
    • Lacerated carotid
  • Treatment:
    • Surgery

Beau was washing the windows at the hospital when the wind picked up, sending him crashing through the window. He was trapped by his safety harness, so they had to call the fire department to extricate him. While they waited, they stabilized his neck. He lost consciousness while they worked to remove him. Once they freed him, he started bleeding from his carotid, so Jules held pressure while they moved him onto a gurney and moved him to an OR. Winston repaired his carotid. While working, Beau developed a clot and lost blood flow, forcing Winston to work quickly to remove the clot. After surgery, his wife was told he was stable, but would need a neuro check once he woke up. His post-op MRI showed no sign of stroke and he had no expanding hematoma. He was later able to be extubated.

Elham Farid[]

  • Diagnosis:
    • Mesenteric tear
  • Treatment:
    • Diagnostic laparoscopy

Elham Farid was hit by a chair when the window washer crashed into the hospital. They got her on a gurney and moved her to the ER. Simone did an ultrasound and saw a small amount of fluid in her pelvis. They took her for a CT. Her CT showed no obvious damage to any of her organs, so they did a diagnostic laparoscopy and found a mesenteric tear, which Richard repaired.
