Full Summary[]
While doing Thanksgiving food prep with the kids' help, Amelia receives a text from Kai informing her that a mechanized injector has solved the cell blob problem. Amelia is delighted and asks if they are working today. Kai replies the video is from last night and wishes her a happy turkey day.
Nick drops Meredith off at the airport. She goes over her checklist of tasks, all of which are completed. He is heading up to his cabin for two whole days off. He invites her. While tempted, Meredith needs to get back to her kids. As soon as he's driven off, she receives word that her flight has been delayed due to the weather.
Nico finds Levi in the cafeteria and hands him a beverage to help get him through the day. Richard chose Levi to work today, which Levi thinks is a precursor to becoming Chief Resident. Myrna then appears with Thanksgiving dinner. Levi is baffled and points out he's working. She'll wait and lay everything out so he can come up whenever he has time. Nico offers to hang out with Myrna but Levi says she'll never set him free.
Richard catches up with Bailey. She came in to work since Dean's dad came by the station and took Pru home with him, thus ruining Bailey's plans. Richard is hopeful that the holiday will bring in good cases.
Owen runs into Teddy. He wants to go check on Farouk but Teddy reminds him of their plan. She'll stay with Megan and Farouk while he goes home to cook dinner with his mother and the kids. They enter Farouk's room. Owen's surgery ran long. Megan says their mother will kill him for being late, which wouldn't be out of the ordinary. Cormac informs the Hunt family that the wound infection is clearing up. Farouk can go home in a couple of days. He has been in this bed for three weeks. Farouk at least wants some cake. Cormac may join him since his kids are with their aunt in San Diego. Owen then pitches to bring the Hunt Thanksgiving dinner to the peds floor for a pic-nic. Cormac will set out to find out boardgames. Teddy and Megan then urge Owen to go to his mother's place.
Richard and Levi are waiting for patients in the empty ER. They hear tires screeching outside. A pregnant woman has parked right in front of the entrance. She hands the doctor a pecan pie. She's a southern girl and never goes anywhere empty-handed on Thanksgiving. She then throws up. She hopes the doctors can check her out fast so she can get back before her kids notice she's gone.
Meredith's on the phone with Amelia as she enters her hotel room. She feels like her mother, missing the holiday with her family. Amelia assures Meredith that she can handle dinner. Zola joins in. She understands Meredith tried to make it. Once the call as ended, Meredith goes back to eating peanuts for dinner.
Link and Scout arrive at Meredith's house. Amelia updates Link on the Meredith situation. Maggie and Winston are stuck in Boston. Link understands Amelia is in charge of four children and Thanksgiving dinner. He decides to stay and help out. Jo took a shift so he was going to spend the day by himself. He didn't want to become his parents and make Scout split the holiday. Amelia says they might as well figure this thing out now.
Jo joins Richard and Levi in the trauma room for Ashley. She is pregnant with her fifth child, 31 weeks along. She is febrile and has abdominal pain. Ashley has been in pain for a few days but she hasn't had time to see a doctor because she's taking care of her kids and in-laws. She thought it was heartburn from the pregnancy. The baby appears to be doing well. Richard finds free fluid. Jo suggests they do an MRI for further evaluation. Ashley doesn't want them to call her husband yet so she won't interrupt the meal.
Meredith finds that all flights have been cancelled. Nick calls her from his car. She mocks him for spending the holiday by himself. She tried calling him after her flight was cancelled but it went straight to voicemail. He informs her the reception's bad. Since he has two hours of road ahead of him, he invites her to keep him company. She accepts. He shares a story of his first Thanksgiving as a resident where a fight between brothers resulted in a sliced-off finger, which was found by the grandfather in the gravy bowl.
On his way out, Owen passes by Bailey, who is admitting Noah to the ER. He passed out working as a bagger at the grocery store. Noah asks Owen for help.
Farouk, Teddy, Megan, and Cormac are playing Monopoly. Megan worries when Farouk starts coughing but he only had a graham cracker stuck in his throat. Cormac is winning. He shares that he used to have a weekly "games night" with his boys. The winner would get to pick next week's pizza topping. Megan corrects him. It's "game night."
Noah's film reveal a pneumothorax. Noah's wife Heather arrives with their son Danny. Bailey informs them that she needs to place a chest tube. Noah apologizes to his son and promises that they'll make the apple pie. Bailey directs Noah's family to the waiting area. Noah asks Owen to take his son somewhere to take his mind off of all this.
While in the MRI, Ashley says she enjoys the quiet. She never has a quiet moment anymore and the kids just keep on coming. She's not on birth control. Ashley screams and says the pain just worse. The images reveal free fluid and possible abcess formation in the right upper quadrant. The pregnancy has pushed her appendix all the way up to her liver. Richard informs Ashley that she needs surgery. Levi will prep her and they will call her husband.
Amelia's basting timer goes off. Link finds that Amelia apparently at some point turned off the oven. Amelia panics since she can't start the turkey in the middle of the afternoon. Amelia wanted to give the kids some normalcy. Link says them fumbling in the kitchen was normalcy for them for months. Link finds mac-and-cheese and bagel bites in the freezer.
Nick tells Meredith that most family Thanksgiving turn into impromptu interventions for his sister. Charlotte learned the idea of Friendsgiving early on. She's at her roommate's. She likes being with big, loud families. Nick says he's close enough to the cabin to picture a fireplace. Meredith's regretting not having joined him. Nick talks about the sound of the frozen lake and how at peace he feels there. Someone knocks on the door of her room. Meredith thinks it's her room service but it's actually Nick. He turned around. She invites him in.
In pre-op, Richard informs Ashley and her husband that they need to perform an open procedure to wash out as much of the infection as possible. Jo will be in the OR to monitor the baby and perform an emergency C-section if necessary. Ashley finds that she needs to consent to a hysterectomy. Her surgeons explain that it may get down to that if there is an abnormal amount of bleeding during a C-section. Ashley then asks Jo to tie her tubes during the procedure. She can't have any more kids.
Teddy calls Owen to ask him to dress Allison in her pajamas since she'll likely fall asleep there. Teddy then hears in the intercom in the background, revealing he's still at the hospital. Owen tells him Noah's back. He'll call her later. Owen then arrives with Danny in the video game room.
Nick asks Meredith what she would have been doing if she were at home. She would have gone grocery shopping and then watch her sisters cook the dinner. Nick likes cooking when he can take his time. Cooking's like surgery to him. You have to know knife skills and how to be in the moment. He's pretty good at both. Meredith's getting better at that. She ascribes that to being hospitalized with COVID. After his transplant, Charlotte and surgery became Nick's only priorities. He's grateful for them. Meredith is grateful that Nick is here with her.
Amelia and Link serve the kids their improvised Thanksgiving dinner. Bailey asks about the turkey. Link says he pardoned it. Ellis asks what that means. Link says it means to forgive, to let go. Bailey then pardons his younger sister for breaking his Buzz Lightyear. Zola is reading a journal and asks Amelia what an AVM is, which Amelia explains to her. Bailey then asks what sex is.
Jo tells Levi and Richard that the baby is having variable decels. Levi inquires about taking the lead but Richard says it's tradition for him to operate himself on Thanksgiving. Richard asks Jo about her Thanksgiving tradition. She used to write the good things that happened to her down on scraps of paper and stuff them into an old takeout box and then read them on Thanksgiving between sad chicken nuggets. The monitor then shows the baby going into distress. She decides to deliver.
Megan hopes Owen will show up with food soon. She never liked Thanksgiving. It makes her miss her dad. But Farouk loves the sides. Teddy admits she is avoiding Evelyn because of Leo. She's worried about how she'll react if Leo shows up in a dress. Megan points out she can intervene but Teddy says she'll intervene so fiercely that it may cost her her relationship with Evelyn. Megan says she has to fight for her son despite the cost. Or she can make Owen do it. Farouk then goes into V-fib. Teddy calls for a crash cart and starts CPR.
In the operating room, Jo delivers Ashley’s baby. After checking in, she returns to the table and begins removing clots and tells Richard, she can’t stop the bleeding.
Teddy is still working on a flatlining Farouk when Cormac enters for an update. She tells him Farouk went into v-fib and his dilated cardiomyopathy is getting worse. She shocks again as Cormac takes over compressions. Teddy assures Megan that they are not stopping, requests another round of epi and charges the paddles once again. Cormac tells them they are not going to get his heart back, that it is just too damaged, but Teddy is confident and refuses to stop. He tells Megan and Teddy the only option is to put him on ECMO. Teddy is his aunt, Megan is his mother, but Cormac is his doctor, and this is where they are now. Megan consents, and Cormac calls for prep of ECMO while Teddy continues CPR.
Levi’s mother is knitting in the cafeteria when Nico comes over to her table. She asks if Levi is on his way, but Nico says he’s still in surgery, he just didn’t think she should have to sit by herself on Thanksgiving. She begins asking where his relationship with Levi is going and mentions that she never saw her only son marrying a man but says it’s every Jewish mother’s desire for their kid be a doctor, second best is for them to marry a doctor. And her, she gets both! Nico is taken aback at the thought of marriage, but Myrna ignores it and asks if they will raise their children Jewish. She mentions that her mother’s name was Beverly, but she went by Bev, a name that could work for either a boy or a girl. That the name is androgynous. She asks if Nico has any genetic markers that they need to be aware of, because Levi’s great-grandfather died of colon cancer. A stunned Nico awkwardly smiles and accepts the plate Myrna holds out.
Amelia is doing the dishes and watches as Link has a living room dance party with Zola, Bailey, Ellis, and Scout. Link smiles as he catches Amelia watching them.
Danny and Owen are taking a break in the video game room. He asks Owen if it’s weird to eat burgers on Thanksgiving. Owen assures him it’s not weird if he likes burgers. He says if you like it, if you’re happy eating it and it makes you thankful, that is what Thanksgiving is all about and it doesn’t matter what you eat. Danny asks if there’s medicine that can help his dad. Owen tells him it’s complicated, but Danny knows his father hurts a lot. Noah can’t play baseball with him anymore, he doesn’t cut the grass, and he breathes funny. Danny's voice breaks as he tells Owen he knows his mom cries in the closet a lot too. Owen tells him Noah is really sick and he knows it’s hard on Danny and his mom. Danny asks if his dad is going to die. Owen assures him that Noah isn’t going to die today, but Danny just says he doesn't want his dad to hurt anymore.
Bailey approaches Heather at a supply cart. She tells her the chest tube is in, Noah is all bandaged up and someone with palliative care is in speaking with him now. Bailey notices Heather has been crying and tells her she can also recommend home nursing aides who can help if it’s too much. Heather stops her saying she can handle bandage changes; she thinks she should have never let Noah take the job at the grocery store. She knew he was in no physical shape to work, but the bills kept piling up. She knows Noah is dying and feels guilty that she’s falling apart in a hallway. She apologizes to Bailey, who stops her and pulls her into a room. Bailey tells her to cry, scream, let it out. It’s unfair. She deserves to feel what she needs to feel. Bailey closes the door as Heather sobs.

Ashley, 40, drove herself to the hospital and vomited right outside the ER. She was 31 weeks pregnant with her fifth child. She had a fever, plus complained of abdominal pain, nausea, and vomiting. She wasn't sure how long she'd been having the pain, but said it might have been more than a week. Jo told her her baby was fine and the fetal heart rate was stable. Richard found free fluid in her right upper quadrant, so they did an MRI. While she was getting the MRI, her pain suddenly worsened. The MRI showed a possible abscess in the right upper quadrant. Richard looked and said it was her appendix, but her uterus had pushed it up by her liver. She was told she'd need surgery. They had to do an open appendectomy so they could clean out the infection. Jo warned her that they might be forced to do an emergency c-section. She had to consent to hysterectomy in the case that there was seriously bleeding. They told her they'd only do it if it was necessary to save her life. Ashley asked Jo to tie her tubes while she was in there if she had to deliver if she didn't have to do a hysterectomy. In surgery, the fetus started having late decels, so Jo did an emergency c-section. They were unable to stop the bleeding, so she then had to do a hysterectomy. After this, they were able to stop the bleeding and she was stable after surgery.
