Researchers and patient safety experts are still trying to answer this important question: how many people die every year from medical mistakes? Mistakes include misdiagnoses, hospital-acquired infections, medication errors, patient falls, and discharging patients too early. As terrifying as it can be for patients to think they might die from a medical error, it's also terrifying for doctors. Even if our mistakes are innocent, they become edged onto our souls like tombstones. And if you never do anything to process these memories, they will come back to haunt you when you least expect it. It's never easy to face our demons. Most of us would prefer to focus on the light. But when we own our shadows, when we name our secrets, when we claim all of who we are, it sets us free. When we lean into what scares us, light comes in.

Haunted is the fourth episode of the nineteenth season and the 404th overall episode of Grey's Anatomy.

It's Halloween night at Grey Sloan Memorial. Meredith and Nick try to spend some alone time together, Levi is stressed due to overworking, and Winston and Owen have the interns practice trauma training on a real cadaver.

Full Summary[]

Owen and Teddy finish having sex in an on-call room. He wishes her a happy Halloween. She scoffs and leaves.

Bailey, dressed up as princess Leia, adds more to Levi's already massive workload. Pru is on her way to come trick-or-treating around the hospital. He's too swamped to properly engage. Bailey puts a pumpkin headband on his head to get him into the spirit. He wasn't invited to Webber's pumpkin carving contest.

Simone finds the other interns watching a security footage video of a first-year resident from another hospital freaking out in a scrub room, demolishing the place. Simone is not a fan of the person in the video losing their privacy. Richard then arrives with a cart full of pumpkins, stating that a carving contest will contribute to the skills needed to be a successful surgeon. Maggie requests an intern for the ER. They all volunteer but Richard chooses Simone.

Winston is eager to get home for trick-or-treating. Maggie brings up his vascular procedures scheduled for tomorrow. Meredith passes by on her way out to take her kids trick-or-treating. Nick announces that they got freshly deceased bodies. While Nick leaves to get the bodies, Meredith goes to fetch Owen while Winston volunteers to help.

Jo and Luna meet Link in the attendings' lounge after their round of trick-or-treating around the hospital. She asks what his plans are for his cancerversary. He was hoping to have dinner with her but she is scheduled for an extra shift. Link then declares this a perfect Tinder night.

Maggie and Simone meet Teddy in the ambulance bay. Two rigs bring in Jarah Al-Hasan and River Epley, two sixteen year olds who jumped off a roof. Jarah missed the swimming people. Both of them took LSD.

The interns are working on their pumpkins. Mika hates pumpkins. The interns are then paged to the sim lab for a trauma.

The interns are met in the sim lab by Meredith, Nick, Owen, and Winston. Nick explains that interns have less and less practice with open traumas due to minimally invasive procedures. They will learn how to do so tonight. Meredith says this is Owen's annual trauma certification session. Rather than dummies, they are working with fresh cadavers today. The scenario is a 32-year-old man with a stab wound to the left hemithorax. Owen pulls out the knife out and fake bloods flows out of the wound. He urges the interns to save the man.

Simone alert Maggie to Jarah's flail chest. Jarah's father arrives. He's confused about Jarah's fall as Jarah went to a friend's house to watch a scary movie. Maggie brings up that he may have taken LSD. Jameel is certain his son doesn't do drugs. When Jarah's sats drop, Maggie moves to intubate while Simone moves Jameel out of the trauma room.

Teddy is examining River in a regular ER bed. River says they jumped off the roof because the bats were looking for them. River appears to be fine. Teddy closes the curtain around his bed and tells him to stay put so someone can come draw his blood.

Owen tells the interns that the airway is secure and a femoral line has been placed. Winston mimics a monitor to support Owen's statement on a plummeting BP. As the interns work on packing the stab wound, Jules wants to name the patient to humanize him. Lucas decides to name Chris. Winston says the best critical thinker gets to join him for an ax-fem bypass tomorrow. Benson goes to perform a thoracotomy but Mika beats him to it.

Meredith and Amelia arrive back at the house with the kids. Nick lets them in with a poor ghost costume. Zola goes to fetch her bag for a slumber party while the other kids go pee. Meredith mocks Nick's scare game. Amelia is babysitting the kids tonight so Meredith and Nick can have a night to themselves. Meredith is nervous about Zola's first night out since the episodes started. Zola comes back with her stuff.

Ben drops Pru off at the hospital. Bailey has her Ewok costume ready. Ben has to return to work.

Teddy informs Levi that there are three kids with abdominal pain waiting for him. The interns are still unavailable since they moved on to trauma training. River's father, Clay Epley, arrives and Teddy takes him to his son while talking about the fall. She then discovers that River has disappeared from his bed.

Link is on his way out to meet Lucinda at Joe's bar for their date. In the ambulance bay, he finds Jo helping a laboring mom out of her car. They then see River standing on top of a rig. He states that he's just like the bats. He jumps off to prove that he can fly, landing on Jo. Link calls for a gurney.

Link is checking out Jo, who likely only has a sprained rest. The patient already gave birth and Jo urges Link to go take care of the patient with the shattered leg. Clay intercepts Link on his way to River. Link says they'll get him an update as soon as possible. Link then joins Teddy in the trauma room. He's not amused because River nearly killed Jo, her patient, and the baby. River has no idea what's going on and is disoriented. Teddy fears he may have a brain injury, or he's just still tripping. Link then realizes he forgot to cancel his date.

Bailey and Pru enter the sim lab. Jules quickly covers up the open wound. Bailey covers Pru's eyes and leaves to take her to Dermatology. While the interns get exposure of the internal injuries with a retractor, Winston tells Owen about his frustration with working under Maggie. She has comments about every aspect of his life lately, not just at work. Owen suggests they go boss the squabbling interns around to make him feel better. The interns locate the source of the bleed in the lower left lobe. Owen says the patient is coding. The interns argue over the next step. Despite Mika's cardiac massage, the patient is declared dead.

Levi exits an ICU room. Nurse Stacy tells him dr. Lawson was looking for him. Levi starts yelling at her about his massive workload until Richard interrupts him.

Nick and Meredith arrive at their hotel room while discussing the interns. Meredith wants to distract herself from thinking about Zola. Meredith takes champagne glasses and invites Nick to join her in the bath.

Maggie and Simone inform Jameel Al-Hasan that they are taking Jarah up to the OR since his chest tube is leaking too much blood. Clay Epley comes up and says Jameel should be arrested since his kid gave River drugs. Jameel thinks it's the other way around. They argue, ignoring Maggie as she tries to calm them down. Bailey succeeds at interrupting them with a speech about parenting a teenagers. The only thing that matters is keeping their kids alive. Clay goes to a different room while Jameel begs the doctors to save his son.

Owen and Winston go over the interns' mistakes. Luckily, in trauma training, there are do-overs. They reveal a second cadaver with the same injury. Jules' family have recently decided to donate their bodies to science, the first generous thing they have ever done. If she had known they were just going to be stabbed, she would not have handled the paperwork. Owen says they aren't monsters. The penetrating injuries were simulated using ultrasound guidance after release forms were signed. This is a generous gift and they shouldn't waste it. Winston pulls out the knife. Round two has started.

In the OR, Maggie finds that the punctured rib has punctured the lung. Simone's phone goes off repeatedly. Jarah's pulmonary artery is lacerated. Maggie grabs Simone's hand and positions them where she needs them while asking the nurse to turn that damn phone off. Simone apologizes. Maggie urges Simone to put her personal stuff aside and get her head in the game.

Levi has finished up carving a pumpkin with a body displaying different types of incisions. Richard hands him another pumpkin. He and Catherine visited several museums on their trip. Seeing all the art was restorative for him. He thinks Levi needs a break. Levi says taking a break means things don't get done and patients will die. He thinks he can't come back again from losing someone he could have saved. He goes back to work.

Teddy urges River to stay still in the CT scanner. Link brings up his own puberty. He doesn't remember LSD being part of high school. Teddy inquires about his situation with Jo. River asks them about Jarah. He fears that he killed him. He realizes his own stupidity and breaks down. Link puts on protection while River asks for his dad. He joins River to hold his hand to keep him calm.

The interns are working together properly this time. They manage to control the bleeding. Benson doubts that this single laceration would cause the patient to be this hypotensive. He urges the others to open their minds. Mika wonders if his dad taught him to side coach like that. Benson never had a dad. After a closer look, Lucas says he found a pinprick hole in the pericardium. They need to do a pericardial window. Kwan takes over to perform it.

Teddy tells River and Clay that he only suffered a minor concussion. They'll observe him overnight and operate on his leg fracture tomorrow. Link announces that Jarah made it through surgery but he still has a long road to recovery. River fears that Jameel won't let them be friends anymore. Clay says they'll find a way through this together.

Maggie finds Simone in the scrub room. She feels something is up with Simone, who has been designated as the intern to watch. She knows Simone was asked to leave her last program meaning the stakes are really high for her here, meaning she wouldn't get this distracted unless something serious was going on. Simone says she was a model resident all the time in the face of blatant racism. Finally, when she raised concern about the treatment of a Black patient, she was told she might be happier at a different program. She was perceived as aggressive. Simone didn't get the chance to file a complaint because she kind of broke when she was asked to leave. She acted inappropriately and the security footage of her breakdown was leaked. She has been trying to get the footage taken down, which is why her phone kept going off. An emotional Simone leaves.

Richard finds Taryn at Joe's bar. Taryn made an anatomic heart pumpkin carving. Richard tells her she doesn't belong here. She counters that she will be walking out of the bar with $800. She knows Levi is drowning but when she lost her residency spot, no one got her a spot in a different department. It'll take more to convince her to come back.

Nick and Meredith are in bed together. Meredith hasn't heard from Zola, which may be a good sign. The principal wants her to skip two grades, which Maggie did and hated. The prep school by the marina focuses on kids who excel academically but Zola is more than that. She needs a whole human approach. Nick says when his dad left when he was 12, it felt like his world fell apart. One day, his mom came home with an RV and they drove it all the way down to the Grand Canyon. He realized that there was a whole world out there where nobody cared that he was a kid whose dad left him. He never would have realized if his mother hadn't shown him that world. Meredith is then called by Zola, who is having another episode. Meredith tells her it's okay. She'll come pick her up.

Teddy meets Link outside in the ambulance bay. The whole world is stupid now. One stupid impulse has ruined those teenagers' lives. Teddy says that can happen at any age. Link admits he's crazy about Jo. She was his best friend and suddenly it became an option. Link didn't want to ruin the friendship and he missed his moment. Teddy thinks it's for the best. He should keep sleeping with other women until those feelings pass. She and Owen were best friends and he used to call her when he ruined his life, but now they are married, which means he also ruins hers in the process.

Owen and Winston inform the interns that the patient is stable. Mika, Jules, and Benson all remind the attendings of their contributions to be chosen to scrub in with Winston. Lucas says he never would have gotten a clear view without the others' work. Owen says he used all the hands around him. He gets to scrub in. Lucas asks if they can't all do it. Winston agrees. He'll e-mail them the case details. Now, they should get some sleep. Once the interns are gone, Owen tells Winston not to let his frustrations grow to the point where the resentment takes over the love that he has for Maggie.

Simone shows Maggie the video. Maggie understands her. As Black women, they are held to higher standards and expected to be on exemplary behavior at all times. Simone deserves a great program and they see her for the good doctor she is here. This video shouldn't be haunting her. River and Clay arrive in the ICU. They want to apologize to Jarah and Jameel. Simone fetches Jameel. River makes his apology and Jameel accepts it.

Richard finds Levi stitching up a patient, which he says is his last patient for the day. Richard admits that they failed him while serving the new interns. This is a new program and they have to own it when they get it wrong. Starting now, all attendings will have to pick up their slack. Richard can't let this program fail again. He asks Levi to hand over his patients so he can go home. Levi complies.

Maggie wakes up Winston in the attendings' lounge. He sat down for a moment after checking on all their patients. Winston admits he is considering changing specialties to vascular surgery. He loves being married to her but he needs to leave cardio if he wants it to stay that way.

Link runs into Jo in the lobby. There was no fracture. She hands him a bag of good candy only for his cancerversary. She thanks him for worrying over her. They'll see each other at home later.

Teddy finds a sleeping Owen in the on-call room. She recalls the year in Baghdad when they missed nine months worth of holidays. On Halloween, he gave her an Easter egg hunt with red, blue, and white eggs decorated with Valentine's day conversations cards. That was her favorite Halloween ever. Owen falls asleep as she talks, frustrating her.

Bailey is packing her bag. Pru has fallen asleep in her stroller.

Simone has told the interns the truth about the video. They are empathetic towards her. They leave to grab breakfast together.

Back at home, Zola apologizes to Meredith. Everything was going fine at the slumber party until they got into their sleeping bags and her thoughts drifted towards Alzheimer's again. Zola wonders when it's going to get better. Meredith says they just need to find her the right school, even if they need to look a little further than Seattle.

He was hypotensive and tachycardic. In the ER, Maggie diagnosed hemothorax and told Simone to place a chest tube. Simone noticed he had a flail chest, so Maggie said he'd need a CT after they stabilized him. When he started coding, they intubated him in the ER. He continued to put a lot of blood out through his chest tube, so Maggie took him into surgery. His pulmonary artery was lacerated, causing catastrophic bleeding, but Maggie was able to stop the bleeding and save him.

Teddy stapled his head while Link examined his leg, which had good blood flow. They took him to get a CT. Teddy said he had a minor concussion, so they'd observe him overnight and he'd have surgery on his leg the next day.
