After a burn victim unexpectedly dies in the midst of a chaotic ER, the Chief and board member Jennings interrogate the residents to find the culprit, as all the Mercy West and Seattle Grace residents defend their actions in an effort to save their jobs.
Full Summary
It's a stormy night in Seattle. While Meredith's voice over talks about how a doctor has to change perspective to come to a diagnosis, a nurse is drawing her blood.
A janitor is cleaning up a whole lot of blood in a trauma room. A bloody axe is lying on a tray.
Alex is in the ambulance bay, checking his phone. Frustrated, he throws it against the wall, destroying it.
A bunch of residents, attendings, and nurses are waiting in a hall, in front of a conference room. Reed is sure someone's getting fired. Charles says it wasn't even his patient, offending Cristina. Alex asks how long this is gonna take. Jackson naggingly asks him if he needs to make another phone call. Alex jumps up and starts fighting him, starting off another fight between the Mercy West and Seattle Grace residents. Mark, Owen, and Derek interfere. Richard comes out of the conference room and yells that it's enough. What happened to that patient tonight was inexcusable, and they're gonna be here until he finds out who's responsible for it. So until then, they're gonna sit down, stay quiet and wait to be called. They all sit down and Richard asks Owen to come in. On his way in, Cristina says she's not going down for this. "Someone is," he says before entering the conference room.
Richard introduces Owen to Larry Jennings from the board and the in-house counsel. Richard needs him to give them the lay of the land of the ER tonight, from the beginning. They were slammed with a mass casualty, a hotel fire. Yang, Grey, Karev, Adamson, Charles, Kepner, Avery, and Torres were working in the ER. Larry asks who was on the patient that died. Everyone, Owen says, as everyone was everywhere. Richard asks who got the chart first. That was Dr. Yang, Owen says.
Now Cristina is sitting in the conference room. She says it was not her patient. She only had the chart for 2 seconds, but then she was called outside. Richard asks her to start from the beginning. She says she was paged to the ER.
In a flashback to earlier that night, Cristina sits down in Meredith's room. Meredith is packing her stuff and waiting for Bailey, who's supposed to sign her discharge papers. Cristina needs Meredith to come back to work soon, as there are more Mercy West residents than Seattle Grace residents, and they're kicking their asses. Cristina gets paged to the ER.
In the ER, everyone busy with the victims of the hotel fire. Owen gives her two charts of patients. She can take her pick, but Bailey needs her to come with her. A voice says they can help, so Yang hands the charts off.
Back in the present, Richard asks her whom she handed the charts over to. She only remembers handing them over, not who she handed them over to.
Now April's in the conference room. She had a chart.
In a flashback, Reed says they can help as Bailey needs Cristina to come with her. April and Reed each take a chart, jealous of Yang, who gets a good case while they're stuck with a crap case. Their patients are Cathy Becker and her son Danny. Reed takes Danny to the bed next to Cathy's. Reed and April start examining them while Cathy assures her son they're all right now. Reed and April talk about how the Seattle Grace residents hate them. April refuses to let them turn her into somebody they can hate as she's easy to like. Actually, it takes a while, Reed says. She hated April at first. April tells Cathy everyting looks great so far. As April asks Cathy to open her mouth so she can check her airway, Cathy is shocked, seeing Charles' patient has an axe in his body. This is proof for Reed and April that they also get good cases. April returns to Cathy and tells her everything is looking good, although she asks Mark to take a look at the possibly third degree burn on her calf. Mark and Lexie come over.
April tells Richard Cathy was a burn victim, so nothing surgical. After handing over the patient to Sloan, April went to go help Dr. Bailey. Earlier that night, Charles fainted while holding the axe steady, so April rushed to Bailey's side to stop the bleeding.
Bailey is in the conference room. She tells them Charles fainted, which she thinks is unacceptable.
Charles explains he fainted because there was a blood drive.
Earlier that night. Charles is giving blood while he and Jackson talk about how the hospital's like a maze. Jackson says the Seattle Grace people are kind of douchey, not helping them find their way. Charles hushes him, as Alex is in the room too. They see his angry stare at them and agree he's a douche. Their pagers go off. While Alex and Jackson run off to the ER, Charles still needs help in order to stop donating his blood.
Bailey and Charles bring in the guy with the axe in his chest. He ran down the stairs with it and tripped. She starts examining the wound and asks Charles to hold the axe steady.
"In retrospect, I should've had a cookie," Charles tells Richard.
Charles faints, hitting himself on the head with the axe. Blood squirts out of the patient as Bailey turns around to see what happened. She then quickly starts covering the wound. April rushes to her side to help apply pressure. While her patient is taken to the OR by April, Bailey asks Alex to clean up Charles' head wound.
April tells Richard that that was the last time she saw the patient.
Lexie tells Richard Sloan didn't treat Cathy's burn; she did. Richard asks where Sloan was.
While Bailey instructs Karev to clean up Charles' wound, Sloan is tending to Cathy's burn. Arizona brings a patient into the ER and requests Sloan's help, so he instructs Lexie on how to tend to the burn. Arizona tells Mark that Evan, her patient, has second- and third-degree burns on 60% of his body. She asks Lexie to come in when she's done with Cathy. Before the door to the trauma room closes, Lexie catches a glimp of Evan, who's in serious pain.
Larry asks Lexie if she felt rushed by Arizona. Lexie assures him she took her time.
Lexie is tending to Cathy's burn. Cathy is in pain, but when she sees and hears Evan moaning in pain, she doesn't seem to mind. Lexie finishes up and bumps into Owen, who's bringing another patient with Callie and Cristina, while on her way to the trauma room. Callie asks Reed for a hand. Reed says she'll be right there.
Lexie enters the trauma room. Evan is shaking heavily and moaning in pain. She sees his body and turns her head away while Arizona is calmingly talking to him. Evan asks her about his dad. Mark takes Lexie apart and tells her burns like this are difficult, so he suggests she switch with someone. She assures him she can handle it and that she's fine here.
As Lexie confirms to Richard she was with the burn victim in the trauma unit, Richard asks her who was with Mrs. Becker. Lexie says Reed was with her.
However, Reed says she was never with Cathy. She was with her son, but she had just finished with him.
Callie asks Reed for a hand. Reed says she'll be right there. She tells Danny he looks fine. Cathy is relieved and asks if he can stay with her until his dad comes, which is no problem as it's a full house already anyway. Reed closes the curtains to give them some privacy and then goes to help Torres.
Patient is a firefighter who fell three stories from a ladder. He has multiple fractures. Roy wakes up and starts asking about the kid he was with. No one answers, as they're too busy examining him. Derek comes over and checks his pupils, and he adds a head CT to the list of tests they need to run.
Reed confirms to Richard that she took the patient out of the ER and to X-ray. The counsel asks Reed if she heard Cathy complain about any pain, but Reed didn't.
Jackson tells them a nurse told him about Cathy complaining of pain.
Tyler comes up to Jackson and Cristina and informs them Cathy is complaining of pain, but neither of them reacts, as she's not their patient.
Richard asks who was in charge of Cathy's case. Reed was never on her case. April was in the OR with Bailey. Lexie doesn't know. She wasn't Cristina's either. Jackson has no idea, although he does tell Richard that Yang ordered pain meds and next time he saw Cathy, she was dying.
In a flashback, Cathy is choking.
Cristina clarifies that when she saw Cathy, she wasn't dying yet, she only had pain.
Tyler gives lies the chart in front of Jackson and Cristina. Cristina asks Alex, who's on his way out to make call, if Cathy's his patient as his initials are on the chart, but he gestures she isn't.
Jackson tells Richard he did ask Alex, but he was on his phone.
Alex tells Richard that he's never even seen Cathy, as he was stitching up Nosedive, the one that fainted. Richard asks Alex again if he was with Percy.
"He was supposed to be," Charles says.
Earlier that night. Alex gestures to Cristina he's not on Cathy's case. He's leaving Izzie a message that he's done calling and leaving her messages and talking to her mother. If she doesn't call him back by tonight, he's gonna report her as a missing person. Charles interrupts him and asks him to help him out with the wound on his head.
Richard wants to know who gave Cathy the morphine then. Alex doesn't know.
Cristina says she did.
As Alex gestures Cathy's not his patient, Jackson remarks that they're all so cranky.
Cristina explains to Richard that because Avery didn't seem comfortable consulting on a patient that wasn't his, so she checked with the patient. She heard her complaints and checked the chart, and then gave her morphine. Cristina is sure. She memorized the chart because she checked it three times. She ordered the morphine, but didn't follow up as her patient came back from CT.
Callie asks for everybody's attention. All burn units in the ICU are full, so the patients need to stay here. Mark comes out of the trauma room and asks everyone to keep the trauma rooms free for the burn cases.
Reed informs Cristina on their patient's lab results while Owen heads off to surgery. Roy hears Evan screaming and recognizes him. Reed says Evan's taken care of and that Roy saved his life, but Roy claims he killed him.
The counsel asks Cristina if the patient complained of pain on or in her chest. Cristina says she complained of pain on her chest, because of the burns. She thinks they're blaming her of giving the wrong medication and asks if the patient had chest pain. Richard says it's hard to tell, as the next thing on the chart is Grey putting in a chest tube. Cristina thought Lexie was with her patient. However, the counsel won't let him say any more and thanks Yang for her co-operation.
Lexie says she was indeed supposed to be with Evan Lang, but a flashback shows that she couln't handle his suffering. She breaks down, so Arizona asks her to step outside and only come back when she's pulled herself together. Outside, she takes deep breaths. Danny, Cathy's son, comes over and tells her his mommy can't breathe. Lexie walks up to Cathy's bed and pulls the curtain aside, revealing a choking Cathy.
Lexie tells Richard that it was a pneumothorax, which is a lung collapse, Richard explains to Larry.
Lexie is counting Cathy's ribs before cutting open her chest to put in a chest tube. Cathy stops her and asks her not to let Danny see her die. Lexie reconcentrates and starts counting.
Richard asks Lexie if she got the chest tube in or not. She was getting there, but she lost count. Richard asks why.
Lexie is counting a second time, but Jackson comes over and blindly makes a hole in Cathy's chest to relieve the pressure.
Jackson explains to Richard that he had to do what he did as Lexie forgot a step. However, she did put in the chest tube.
Jackson tells Lexie that it was a tension pneumothorax, so you have to relieve the pressure before putting in a chest tube. Lexie tells him she's got it.
Jackson tells Lexie indeed put it in the chest tube, although she seemed a little distracted.
Lexie explains she was distracted because Yang was making a toddler cry.
A flashback shows Cristina yelling at Danny to stop and asking him what he's doing. Lexie sighs and returns to putting in the chest tube.
For the record, Cristina wants Richard to know it wasn't a toddler, as he was 5 already. She's sure Richard would've yelled at him too. Richard asks why he was there in the first place.
Reed has no idea how he got there. She mentions the patient was coding, which surprises Richard.
Owen and Cristina are about to go to the OR with Roy. They'll call Torres and Adamson when they're ready for them. Callie informs Roy his crew is in the waiting room; they'll let him see them after. They say he's a hero, but Roy wants them to tell his crew that he's sorry. He utters he almost had him, he felt his fingers in his hand, but then he missed a step. It was a stupid mistake. As he continues to panic, he suddenly goes into V-tach. Reed starts prepping the paddles, but Cristina thumps Roy's chest, fixing the problem. Reed lays the paddles down and behind their backs, Danny takes them into his hands. Cristina notices it and yells at him to stop. He starts crying.
Richard asks Cristina who was watching that kid, which is exactly what she said.
Lexie says she took the kid. A flashback shows Lexie telling Cristina that Danny was with her, but she's supposed to be in trauma room 2. She goes back to the room, but upon hearing Ethan's screaming and seeing him suffer, she returns and takes the kid to the waiting room.
Lexie tells Richard she left Cathy because she was stable, and she was supposed to be with the boy. Not Danny, Ethan.
Earlier that night. Lexie is talking to Danny in the waiting room as Arizona comes over, angry. Lexie says she's supposed to sit with Danny until his dad comes. Arizona takes her apart and asks a nurse to sit with Danny.
Arizona is angry with Lexie for leaving Ethan alone. Ethan is feeling everything as his nerves are exposed and raw and they have to make him feel even worse before they can make him feel better so Lexie needs to shut down her feeling and talk to Ethan about future, remind him that he still has one behind all the pain. If she can't do her job, Arizona needs her to find someone who can and send them to her.
Lexie tells Richard she never saw Cathy again, nor did she see Karev perform an emergency cricothyrotomy.
Charles did see it, as he was right there.
As Arizona walks off after her speech, Charles approaches Lexie and asks her if she's seen Karev, who's supposed to finish stitching up his head wound. Lexie stammers she hasn't and Charles continues his way to the ER to look for Alex. However, nurse Tyler summons him as Cathy is not breathing. Charles gets ready to intubate, but the airway is completely closed off.
Charles says he was having trouble intubating. Reed confirms Cathy's airway was completely closed off. She couldn't tube her either.
Earlier that night. Reed joins Charles and asks for a smaller tube, but she has trouble too. Alex comes over and tells them to move.
Lexie tells Richard what happened. She did see what happened, but she didn't help out as Ethan needed her.
Back in the trauma room, Lexie sits down with Ethan and starts talking about the raining in Seattle, which she likes.
Lexie says she went back to her patient as Karev seemed to have everything under control. Reed, on the other hand, thinks he was totally out of control. He was pale, shaky, just really unsteady.
Alex gets ready to perform a crike, going in blind, which shocks Reed. As he's about to cut into her neck, he mumbles something about the carotid.
Reed thinks he said something about hitting the carotid. Larry and Richard share a shocked look.
Alex makes the cut.
Richard asks Alex if he gave blood today, given Reed's description. He says they all gave blood, but he didn't give blood today.
While he's being prepped to give blood, Alex is on the phone with Robbie, who claims she hasn't seen Izzie in a while. He asks Robbie to call him when Izzie calls her. Alex overhears Charles and Jackson calling him a douche, after which all of their pagers go off. Alex asks the nurse not to stick him as he has to go. He apologizes and runs off.
Richard asks him what the problem was then. He claims there wasn't any problem.
Reed says there was a problem. She came back from seeing the firefighter up to surgery.
Earlier that night. As Reed enters the ER, one of Roy's colleagues approaches her. He and his colleagues want to honor Roy when he comes out of surgery. Reed notices Charles waves at her to come over as he's trying to intubate Cathy. She apologizes to the firefighter and walks up to Charles.
Charles says he wasn't waving at Reed, he was waving at Alex, who wouldn't get off the phone. "He what?!" Richard says.
Charles has trouble intubating and waves at Alex, who's finally managed to get Izzie on the phone. He wants her to come home so they can talk.
Alex tells the committee he hung up the phone. Richard asks what was so important. Alex insists he hung up the phone, came over and criked her. Only after another doctor got there, and that was while a patient was dying, Richard blames him. Alex yells he hung up the phone.
Alex is stitching up Charles' wound as his phone rings. It's Izzie. He walks off to continue the conversation. He moves further away to make room for Lexie and Arizona, who's giving her speech to Lexie. He's found a spot in a supply room, from where he sees that Tyler's asked Charles to come over. Alex asks Izzie to come home so they can talk, but she only says she's not coming back yet. He asks if this means she's not coming back yet, or not at all. Meanwhile, Charles waving at him. Alex gestures to be patient, while Reed goes to help out Charles. Alex asks Izzie to give him a number so he can call her later. He repeats the requests several times, but she doesn't give him the phone. As he notices how bad the situation is getting, he's forced to hang up the phone and help out Charles and Reed. As he's going in blind, Alex softly repeats to himself not to hit the carotid and then performs the crike.
Alex tells the committee he got there in time to perform the crike. He hung up the phone. After that, he stayed with Cathy. She was stable, as it was a good crike.
A flashbacks shows him sitting by Cathy's side while checking his phone.
Then, out of the nowhere, they started to lose her. She was sinking fast.
Alex asks Reed and Charles to help out. He wants Hunt paged and the labs he asked for.
April says she was shocked when she saw the labs.
A nurse asks April if she's A.K., the initials that are on Cathy's chart. April says that's Alex, but when she sees the labs, she runs off.
Jackson says her carbon monoxide level was 28, and she was spiraling down.
While Alex and Jackson are working on Cathy in a trauma room, Charles is in the phone with Hunt to update him on Cathy's status. April enters the room.
Cristina tells Richard they needed more hands.
Earlier that night. Cristina and Owen are in surgery. A nurse is holding a phone to Owen's ear. He's giving instructions to Charles. Owen tells Cristina to scrub out and go see if they need her help.
April says there was so much swelling that they couldn't find good veins, so they had to start a central line.
The residents continue to work together to save Cathy's life, but she's circling the drain.
April says all of her organs were failing.
When Cristina got there, Cathy was coding.
Alex shocks her, but Reed notices there is blood in the tube. Cathy is bleeding out.
By going over all what happened, Reed suddenly says she knows what happened. She knows why Cathy died.
The residents are fighting over how they can still save Cathy's life. There's blood everywhere. Derek comes in. He yells that it's over. They've lost her. As he asks someone to call time of death, they realize she was no one's patient.
Reed gets out of the conference room. She visibly feels bad. April gets up to ask what's wrong. Reed says she's sorry as Richard comes outside to call April inside. April asks Reed what she told them. Reed doesn't reply and just walks off. April walks into the room with the Seattle Grace residents curiously staring at her.
Richard tells April he doesn't know her, but Adamson just spent 20 minutes telling them what an excellent doctor April is. Everything confirms this, and Richard believes she is good. So, she needs to be able to tell them why Cathy died of multi-system organ failure. April goes over the case and concludes there must have been more damage than the burn as her lung collapsed. Richard says that's right. She needed a chest tube, which Lexie put in. But he wants to know why Cathy's lungs would have failed. April head Cathy went into respiratory distress and that they had to crike her, but she wasn't there. Richard asks why she needed a crike and not intubation. April says her throat was too closed off. Richard asks what would cause such inflamation of the throat. Soot, smoke inhalation, April says. Richard asks April how Cathy's throat looked. April can't remember. Richard repeats the question and suddenly April realizes what happened.
When she was about to check the airway, they were distracted by the guy with the axe in his chest, and she forgot to check Cathy's airway after the distraction.
Richard concludes that April did not look in Cathy's throat. If she had, she would've seen soot and then she would've intubated immediately and then Cathy wouldn't have died. April stammers she was distracted by the chaos. Richard understands the mistake given the circumstances. Larry subtly reminds Richard to stick to what they discussed. Richard apologizes, but her small mistake lead to a woman's death.
Earlier that night. As Cathy's no one's patient, Derek calls time of death.
Richard tells an emotional April that she's fired.
Earlier that night. The doctors stand in silence next to Cathy's body. Derek leaves the room. In the other trauma room, Lexie is still talking to Ethan.
Danny is still with the nurse. His father arrives and Danny happily jumps into his arms. Danny's father asks where his mommy is.
Derek is sitting in the conference room while Richard is packing his stuff. Derek thinks it's not the doctors, as they're all good doctors. Richard knows that. For Jennings, this was all about legal, but it was actually for him. He needed to find out who was responsible. At least they were able to do that. Derek says it may not be one doctor. Maybe it are too many doctors, who don't know and trust each other. When he got to that room, there was chaos. That's the system now: chaos. That's the system that's been in place since the merger. It's Richard's system. So, Derek advises him to look again at who's responsible. Derek leaves Richard alone to overthink the matter.
Alex is in the ambulance bay, trying to call Izzie, but she doesn't answer. He throws his phone against the wall.
Roy is out of surgery. His colleagues are lined up in the hallway, complimenting him and showing their support.
At the other end of the hall, Reed and Cristina watch how close the firefighters are. Cristina asks if April's gone already. Reed rolls her eyes and says April is a better doctor than she is. She's frustrated and asks Cristina to shut up. Jackson comes over and asks Reed if she's talked to April, but she hasn't. Alex walks by and Jackson stares at him, trying to see if Alex is going to hit him again. Alex apologizes for that. Jackson understands it was a rough night. Charles and Lexie come over. Everyone but Reed agrees April made a pretty stupid mistake. Reed says she got distracted and made a small mistake. Charles and Jackson say it got her fired, and Alex adds it almost got all of them fired. Cristina sides with Reed and says that that's their job after all. They all have made mistakes or been distracted today. However, their patients didn't die and that's why they didn't get fired, but it could've happened to all of them. April walks past them on her way out, carrying her stuff. She ignores them all as they try to make contact.
Bailey walks into Meredith's room and they both sign the discharge papers. Bailey leaves as Derek appears in the doorway. Meredith's happy that she finally can go home after having waited all day for the discharge papers. She sits down in a wheelchair and Derek wheels her out of the room. While Meredith's voice over says that a new perspective may give a doctor a whole lot of new questions, Meredith asks him what happened today.

Cathy came into the ER after a hotel fire. Cristina got the chart, but handed it off right away to April as she was called to another patient. She had burns on her chest and calf. Before April could check her throat, she was distracted by a patient with an axe in his chest. She said the burns on her chest were minor, but the one on her calf might be third degree, so she had Mark come look at it. Mark determined it was deep second-degree burning and left Lexie to treat it. After her burns were treated, she started complaining of pain from her chest burns. Cristina prescribed pain meds for her. Later, her lung collapsed, so Lexie went to put in a chest tube. Before she could, Jackson came in and relieved the pressure so she wouldn't die when Lexie put the tube in. Later, she stopped breathing. Charles and Reed tried to intubate, but they couldn't, so Alex did an emergency cricothyrotomy. She was stable, but then she got pulmonary edema. They worked hard. They have her pure oxygen and started a central line. Her organs all started shutting down. She started bleeding out and they weren't able to save her. Her death was traced back to April Kepner not checking her throat during her initial exam. There was soot in her throat, which is what started the chain reaction that led to her death.

A man was injured during a fire while carrying a fire axe down the stairs. He ended up with the axe in his chest. They brought him to the ER where they accessed his injuries while keeping the axe stable. However, when Charles fainted, he pulled the axe out as he fell, which caused the wound to start bleeding profusely. Bailey stopped the bleeding with April's help.

Roy, 56, was a firefighter who fell during the hotel fire. He had fractures to his hip, femur, and radius. He had uneven pupils, so Derek ordered a head CT along with his other tests. His scans showed a liver lacerations and possible kidney hematoma. They took him into surgery, but he coded on the way. They were able to revive him and took him to surgery, which was successful.

- Meredith gets discharged from the hospital at the end of the episode. She gave a piece of her liver to her father in Tainted Obligation.
- This was supposed to be Sarah Drew's final episode as Dr. April Kepner, but on the day of broadcast of this episode, Sarah received news that the show wanted her back.
