“ | In 1986, Congress passed the Emergency Medical Treatment and Labor Act, which prohibits emergency rooms from denying patients care. If you don't have a dollar to your name, it doesn't matter. You can go to any emergency room at any time and they have to treat you. Some people think this law has contributed to overcrowding and hospital closures. But I don't know a single ER doctor who would turn a patient away even if they could. Sometimes all that means is treating an ear infection. Other times, you see a patient on the table and your heart stops. The rarest of diseases. Hopeless co-morbidities. Cases that you've never seen that you're not trained to handle. All you can do is close your eyes and say a prayer. Because once a patient enters the emergency room, they're yours. When you're met with the impossible, the only place to turn is toward each other. When you lose, you have someone to cry with. When you walk into the unknown, you have someone to hold your hand. And when you win, there's someone by your side to celebrate. And suddenly, the impossible feels possible. But sometimes, there's no one to turn to. Sometimes, you're all alone. In those times, you have to keep the faith. Remember that even if you can't see them or feel them or hear them, your people are out there, waiting for you. Even after you're gone. | ” |

James says he could come visit for a long weekend and says Levi will be back for his mom's birthday. Levi says his mom is actually going to come to San Antonio to see him so she can see where he's living. James says there's the Fourth of July, but tickets will be expensive. James quotes scripture to console Levi, who says San Antonio feels far away. Levi finds it attractive that he can quote scripture like that.
Teddy asks Owen to talk about what happened. He says he knows enough and walks away.
Lucas asks Ben if he can play baseball. Ben says he pitched in college. Lucas asks if he's free on Saturday. Richard asks if he's re-starting the hospital softball team. He's trying to, but Simone asked if she could use a tee and Jules said she would hit, but not run. Ben can't do it because he has to take his kid to a birthday party. Lucas asks if Bailey could do it, but Ben says he'd have to be the one to ask her. Teddy says there are two incoming traumas and says she'll meet them outside. Bailey finds Ben and asks him how last night went. Ben says Tuck's team won by two and Pru got a visit from the tooth fairy. Bailey saved a patient, but lost a resident. But if Levi's happy, she's happy. She sees the ambulances and tells Ben she'll see him at home. J. Gause announces a 22 year old woman with crushes injuries to her legs. Richard recognizes Chloe, who is complaining that her legs hurt. Chloe asks about Mika and they promise to find out for her. Lucas sees the other ambulance and realizes it's Mika. Bailey jumps in to help Teddy. Mika asks about Chloe and they take her inside.
In the ER, Bailey observes bruising on Mika's abdomen. She and Teddy examine Mika and order labs. Winston comes in. Lucas tries to step in to help, but Bailey orders him out and Mika tells him to take care of Chloe. She starts to code, so they intubate.
Lucas goes to Chloe. Richard tries to send him away, but Lucas says he told Mika he'd help her sister. Richard agrees to let him help. Link comes in and looks at the x-rays. She has multiple fractures and he's worried about blood flow to her legs. Lucas goes to get the doppler and meets the other interns, who have just arrived. He sends them to Mika.
When Bailey sees the interns, she tells them to step away. Mika has bleeding in her abdomen. Jules tries to help, but Bailey says she can't help. Benson and Simone have to pull Jules away as they start compressions on Mika.
Levi says they'll be fine. People do long distance all the time and he's always wanted to work with kids. He's never lived anywhere but Seattle. He needs to get out and experience something new, like Ruby Garner, who is wearing a NICU graduate onesie and cap as she does her car seat test. Jo tells Levi his speech needs work. Jo asks if Ruby's going home, as Jo helped deliver her. Levi says she is if she can pass her car seat test. Jo can't believe Levi is leaving her. He says he'll be back all the time, but she knows that'll be to visit James. He asks if she thinks it's a mistake to do long distance. It's already hard and he hasn't even left. Jo is in no position to give relationship advice. Levi's phone chimes and he tells Jo that Mika was brought into the ER after a car accident. Just then Declan and Kelly Garner come in. They're early, but they ask how Ruby's doing. Levi tries to tell them she's doing well, but then notices she's turning blue. He pulls her out of the seat and calls for an intubation tray. Jo has a nurse take her parents out of the room so Levi can intubate.
Owen comes in as Teddy and Winston are preparing for a thoracotomy on Mika. Before they cut, she stabilizes. She has a diaphragmatic rupture and her liver has herniated into her chest. They decide to move the OR right away. Bailey sees the interns watching and says she'll meet them in the OR. She exits and has the interns come with her.
Link says neither leg has a signal. Richard says she just had her first round of chemotherapy, so they need to watch for acute kidney injury. They diagnose compartment syndrome and do bilateral fasciotomies to relieve the pressure.
Levi tells Jo Ruby is breathing again. Her parents are worried, but Jo is more jealous that their baby is born than sympathetic. Levi consoles Ruby as he does an ultrasound. She has fluid around her heart. He'll need to do labs and aspirate. Jo offers to get the parents' consent for him. Levi says he doesn't know what he'd have done all these years without her. Jo tells him to stop because she refuses to cry before lunch.
Bailey updates the interns on Mika's condition. She'll try to keep them updated, but Mika's their priority. She promises to do whatever they can and goes back inside. Jules tells the others that Mika helped her on the night shift and that's why she was so tired. Benson tells her not to think like that. They start to fight over it, with Jules not believing Benson cares.
Simone runs into Lucas on the stairs. Lucas says it's brutal, but at least he gets to help. Simone just has to watch. She can't find Jules, who also won't text her back. Lucas tries to get her to slow down, but she tells him to keep her posted on Chloe and walks away.
Owen and Teddy scrub in for surgery. Teddy drops the soap. She shares a memory of Mika stealing the donuts after a meeting. Teddy feels guilty about not helping her when she was struggling. Owen is sure Mika forgives her. Teddy then says she's sorry about Cass. Owen tells Teddy not to think about that. He tells her they can do it.
Chloe is getting a CT. Ben says Richard is trying to get a hold of their parents. Lucas says Mika is the glue of their class. She's best friend with everyone. Ben knows that as well. Lucas shares a memory of Mika getting herself ready for a night shift. Ben says it's harder when you care about your patients. Link and Richard come in just in time to see the scan come up. Richard sees an irregularity in the popliteal artery. They'll have to get a vascular consult when they open her up. Link says her fractures will need external fixation. They need to prepare her for surgery.
Levi marks his target for the needle aspiration and then starts. Levi says once they're done, Ruby and her parents will feel much better. Her parents love her even though they have lived in different places. Jo asks if he's comparing them to a NICU baby and her parents. Ruby becomes bradycardic, so Levi gets started. Levi sings to Ruby as he inserts the needle. Jo reminds Levi of when he dropped his glasses inside a patient and says he's come a long way since then. Ruby stabilizes and Levi says Jo can tell her parents she'll have to stay for a few more nights, but then she should be able to go home. Jo tells Levi long distance is awful. She'd never recommend it, but if he's committed to making it work, it will.
Simone finds Jules and offers her snacks. Jules declines and says she came down there to be alone. Simone says Mika's not just Jules's friend, but she's acting like she's going through this alone. Jules says they're not just friends. Simone asks if they're seeing each other. Jules isn't sure. She explains what's been happening and says it's complicated.
Mika is losing a lot of blood. Her liver is fractured in the middle. They want to try a Pringle Maneuver, but she's still bleeding. Winston wants to remove the liver, but Teddy is against it, because liver failure will complicate her recovery. Owen agrees with Teddy. Bailey tells them to stop arguing so they can save Mika's life. She says no to removing the liver. She wants to pack Mika and get her to the ICU while they think of a better plan. If they disagree, they can leave.
Benson checks on Mika. Teddy says she's putting up a good fight. Benson offers to sit with Mika so Teddy can leave. Once they're alone, Benson tells Mika she messed up the laundry again. He has no clean socks. He tells her she sucks at laundry. He promises to do her laundry for as long as they live together if she pulls through. But he can't do everything for her. After thinking of the other things she doesn't do, he says she's a terrible roommate.
Winston finds Bailey on a treadmill in physical therapy. It helps clear her mind. He asks if she has any ideas. She says no. They all have a lot of experience, so they should have ideas. Winston tells Bailey about Mika's surgical skills and they talk about their shared enjoyment of teaching. Bailey suddenly stops. She has an idea. Winston follows her out.
Only 25% of Mika's liver is functioning. That's not enough to preserve function, but Bailey wants to resect her whole right lobe and the parts of the left that are damaged. Then they'll give the blood supply she needs to recover and re-grow her liver. Teddy says the common hepatic artery is too damaged for that. Bailey says they'll remove her spleen and use its vessels to graft it. Owen asks if she thinks the remaining liver will function. Bailey hopes so. It's a long shot, but Mika is strong and she gives everything she's got. They agree to try the plan.
Levi finds James in his office. He says it's graduation day for the preemies. James asks if he can help Levi. Levi asks James to move to Texas with him. He's lived all over the world, so why not San Antonio. James says his whole life is in Seattle. He just bought his loft. Levi says he's falling in love with James. He says James doesn't have to decide right away. He can just think about it. Levi gets paged and James say he'll meet him in the lobby after work.
Simone and Jules look at the babies, but they don't make Jules feel any better. Simone brings Jules into the room with her and they sit in some very comfortable chairs. Jules decides she likes that. Simone asks how long she and Mika have been a thing. Jules says they haven't had sex yet, but that's not what Simone is asking. Jules says it's been a couple of months. They keep trying to slow down, but keep picking back up. Jules has never had a relationship that feels like this. It's easy and electric and they're both scared of messing it up. Simone says Mika won't go down without a fight, especially with Jules to fight for.
Richard says Chloe just started chemo and now she's going through this. The popliteal artery starts bleeding. Lucas leaves to get someone from vascular to help.
They start Mika's surgery. The liver is still bleeding. Lucas comes in and asks for Teddy or Winston for Chloe. Teddy says she'll go. Lucas looks at Mika before he leaves.
Out in the hallway, Lucas pulls off his mask and vomits into a wastebasket.
Teddy steps in to Chloe's surgery. The blood flow to her leg is compromised. Lucas comes in as Teddy gets started.
Bailey, Winston, and Owen keep working on Mika.
Levi brings the waiting interns coffee.
Link finishes Chloe's fixation and Lucas bandages her.
Winston finishes Mika's surgery. Bailey removes the clamp and it holds. They need to pray that it works.
Jo finds Levi in the lobby. She asks about Ruby, who is improving. Jo asks where James is. She says he might have gotten held up with a patient, but Levi doesn't need Jo to console him. He just freaked James out. They'll be back to long distance. Jo says getting drunk and sleeping with Levi is the best mistake she ever made. She asks him to be her babies' godfather. When she's upset, he's her first call. On the worst days, he makes her laugh. She hated when he quit OB because she loved working with him. Now she's losing him again. Levi says he'll talk all the time. Jo says the world is hardened, but he walks around with an open heart. She wants him in her kids' lives, so he can teach them to be gentle and teach them about Star Wars. He says he's flattered, but he's Jewish. She encourages him to say yes and he does.
Benson admits he didn't like Mika when he first met her. She was just loud and in his face all the time. He likes calm and quiet. Jules says she met Mika right after bumping into someone she'd slept with and Mika rescued her. Lucas asks who, but that's not the point. Simone remembers and asks if it was Link. Jules is glad she didn't run into them that day. Bailey comes in and tells them Mika is stable and recovering in the ICU. Now they have to wait and see.
The interns sit in Mika's room. They talk about paperwork and how Mika actually loves it. They hear a code blue. It's Chloe. They all race out.
In Chloe's room, they run the code.
Owen asks Teddy how Mika is.Teddy says she's good. The interns are with her. Owen tells Teddy he overreacted. She didn't do anything wrong with Cass. Teddy's still sorry. She doesn't want to cause Owen pain. They go home together.
They continue working on Chloe. Link and Ben come in. It's been forty minutes. Link tells Lucas he's done everything he can. Lucas stops and calls time of death. Jules says they can't just stop. She re-starts compressions. Benson finally gets her to stop. Jules runs out of the room.
Ben finds Bailey in the lounge. He has Chloe's personal belongings. Bailey realizes she didn't make it. the crush injuries on top of chemo were too much for her body. Now he has her things for Mika when she wakes up. Bailey says if she wakes up, then says she needs to be more positive. She doesn't want to cry. Ben asks if she's staying. It'll mean another night away from him and the kids, but Ben tells her not to worry about that. Bailey tells him the kids can have whatever they want for breakfast. Something good needs to happen.
Simone finds Lucas outside. He says Mika told him to take care of her sister. Simone says he did everything he could, but he still blames himself. Simone says Mika will understand. Lucas walks away.
Levi goes to his bike and unlocks it. He looks back up at the hospital and remembers his time there. James comes to him apologizing. Levi says it's okay. It was a big ask and he knows James wasn't ready. James says he missed their meeting because he was in the administrative office, resigning. He asks if Levi still wants him to go to Texas. Levi kisses him. James says he hopes that's a yes because he just quit his job. Levi says yes and they leave the hospital together.
The interns wait in Mika's room. Bailey comes in and tells them to go home. It could be days before she wakes up and they need to get rest. Benson asks her to let them know if anything changes. She promises to call as soon as it does.
Time passes as Mika remains in a coma. People come in and out of her room.

Mika, 27, was unconscious in her van after her accident. She was extracted and moved to a gurney, then taken to the hospital. She had blunt trauma to her abdomen. They ordered labs and a chest x-ray, then put in a chest tube. She was bleeding internally, so they gave blood transfusions and then did a thoracotomy in the ER. Her diaphragm had been ruptured and her liver had herniated in her chest. Once they stabilized her, they moved her to the OR. Her liver was fractured in the middle. Winston wanted to remove it, but Teddy said liver failure would make her prognosis worse. Bailey stopped their arguing and decided they'd packed her and get her to the ICU so they could have the time they needed to figure out a better plan. Bailey thought about it and decided they'd resect her right lobe and the damaged parts of the left, then use the vessels from her spleen to give her live the blood supply it needed to recover. It was successful, though Mika remained in a coma for a while after surgery before waking up.

X-rays showed multiple fractures to her legs. She had no blood flow to her legs, so they did bilateral fasciotomies to relieve the pressure. She then had a CT, which showed no chest or abdominal injuries, but irregularity in her popliteal artery. Link planned ORIFs for both legs. During her surgery, she started bleeding heavily from her leg, so Teddy stepped in to repair the bleed, saving her leg. She was stable in the ICU after surgery, but then she coded and was unable to be resuscitated. She was pronounced dead at 22:16.
