“ | In the early days of medicine, when most diseases were untreatable, what patients wanted to know was how long they had left. People believed the best doctors weren't the ones who could help you buy more time, but the ones who made the most accurate prognosis. Now that we have more tricks up our sleeves, we're less interested in calculating the odds and more concerned with beating them. But just because we don't spend as much time predicting the end, doesn't mean it's not coming. If you only had ten months to live, what would you do? Would you hold your people close? Or would you let them go? Throw caution to the wind? Or turn back to the safest, most comforting place? Because it might be the end of the road. Or it could be just the beginning. | ” |

Medical Notes
Dylan Gatlin
- Diagnosis:
- Locked-in syndrome
- Doctors:
- Amelia Shepherd (neurosurgeon)
- Lucas Adams (surgical intern)
- Simone Griffith (surgical intern)
- Treatment:
Dylan was locked in, only able to communicate through blinking. She had daily MRIs to monitor her brain activity. Her MRI showed no change in her hematoma. Amelia advised her parents they just needed to wait for her body to absorb the blood. When Dylan stopped blinking to respond, her parents were told that sometimes, things got worse before they got better.
Nora Young
- Diagnosis:
- Aortoenteric fistula
- Infection
- Doctors:
- Teddy Altman (cardiothoracic surgeon)
- Winston Ndugu (cardiothoracic surgeon)
- Ben Warren (surgical resident)
- Benson Kwan (surgical intern)
- Treatment:
Nora was back in the hospital because her graft was infected. Her blood cultures were growing fungus and her CT showed a new aortoenteric fistula, meaning they needed to remove the source of the infection, her graft, as soon as possible. Winston worried she was too unstable, so Teddy suggested diverting blood flow away from her aorta to give her time to recover. They took her into surgery, but her heart stopped before they could get started and so they decided it was unsafe to proceed. They consulted with palliative care and told Nora her prognosis. However, Teddy later had an idea that could possibly save Nora's life and went back to the hospital to review her scans.
Deb
- Diagnosis:
- C. diff
- Toxic megacolon
- Perforated colon
- Doctors:
- Richard Webber (general surgeon)
- Jules Millin (surgical intern)
- Treatment:
- Antibiotics
- Colectomy
- Ileostomy
Deb was in the ER. She claimed she had a sharp pain in her arm and said she was worried because her mother had died of a heart attack four years prior. Once her date was gone, she admitted she was really there for diarrhea and cramping severe enough to cause vomiting. She'd had the diarrhea for five days and thought it was due to the antibiotics she was taking for a UTI. They got a stool sample and then took an x-ray of her abdomen, which showed toxic megacolon. They started antibiotics and admitted her for tests, which confirmed c. diff. When her colon perforated, they took her into surgery, where they removed her colon and then did an ileostomy. After surgery, she was told the nurses would teach her to care for her ileostomy.
