Oxygen is the human body's fuel. It powers our brains, organs, and bodily tissues. Without a proper supply, our bodies can't function. Our brains, hearts, and kidneys can only tolerate a lack of oxygen for a limited time before their cells begin to die: about five minutes for the brain, about thirty for the heart and kidneys. When a patient loses oxygen, doctors know the clock is ticking. When a patient codes or loses their airway, a doctor's first priority is to get oxygen flowing. We have a limited time before damage sets in, so we spring into action. But in life, time limits aren't as clear. It can be hard to gauge whether it's better to hurry up or take things slow. We don't always know how much time we have before the clock runs out on opportunity. Often, we can't even be sure if our time's passed. All we can do is cease the present.

Ready to Run is the eighteenth episode of the nineteenth season and the 418th overall episode of Grey's Anatomy.

Full Summary[]

Simone puts on her wedding dress in her bedroom. As she's on the phone with Trey, Mika is making some alterations. Simone wanted to rock a jumpsuit but her grandmother pulled her mother's dress from the attic. Lucas comes in and is stunned for a moment. Richard e-mailed about a special announcement. Mika warns Lucas that they are leaving in five minutes.

Taryn meets Levi at the coffee cart outside. She agreed to come in to talk to a few people.

The surgical staff gathers in the lobby. Richard and Teddy announce that there are two nominees for the Catherine Fox Award: Nick for his work on xenotransplantation and Winston, who is co-nominated with Maggie for their partial heart transplant. They look forward to celebrating them at the Boston ceremony.

As the crowd disperges, Richard approaches Nick and Winston to congratulate them personally. He saved them seats on the Foundation jet. Winston is truly honored. Amelia congratulates him as he heads to the OR but Winston gives her the cold shoulder. Richard tells Nick he'll have more information on the travel plans later today but Nick says he's not going.

Mika presents Sam Sutton's case. He's day 2 post-op. All Doppler signals are strong and there are no signs of infection. Owen and Link are writing a case report about Sam. Sam asks what happened to the other doctor, with Disney princess eyes. Owen guesses he's talking about Jo. Sam checks if she's actually single or just told him that she was. Owen says Jo's with other patients today; Mika will tend to him today.

Jules intercepts Bailey and Benson as they are about to round on Maxine. She wants to be on Bailey's service since she can provide a high level of care on this case. She assures Bailey that she can be professional. Bailey agrees and sends Benson to find Amelia. Bailey and Jules then enter Maxine's room. She is doing much better and will be moved to a regular room later today. Maxine tells Jules that in case it happens again, she wants to be cremated and if her time to go has come, she wants no extraordinary measures. She wants to go out as the vibrant woman that she is. Jules says she's not dying. Bailey says they'll get her the paperwork to make everything official. Maxine has been having a good time here, especially with Jules sleeping down the hall. Jules claims she had some late nights.

Teddy rushes down a hallway as Simone and Lucas try to keep up. They're on their way to see Ray Sanchez, a 29-year-old patient with an abdominal aorta aneurysm, which is very rare below the age of 60. She quizzes the interns on complications. They enter the pre-op room and find Ray putting on his clothes. Teddy can't believe it's happening again. He tells her he can't do this and rushes out. The doctors run after him.

Jo finds Link and tells him about a disappointed look she saw on Scout's face that reminded her of Link. Nurse Stacy comes up to inform Jo that Sam was asking for her. Jo then sees the look on Link's face and asks what's going on. Link thinks it's inappropriate to tell patients that she's single. Jo explains that he asked when thinking he was dying. Jo appreciates his misplaced chivalry but she can handle being hit on.

Teddy reminds Ray they have had three failed attempts at surgery over the course of two months. Ray knows that he likely won't survive if the aneurysm bursts but all the complications on the consent form scared him off. Teddy stresses there is a 10% chance of a rupture. Ray doesn't want to hear any more statistics. Being an illustrator doesn't require much exertion. He'll take his chances. He gets on the elevator. Teddy hopes his mom will once again work on him to make him come back. She'll page the interns when she has another case. Lucas tells Simone he'll catch up later.

Jo arrives in Sam's room. He admits he wanted to see her. Post-op is so boring so he wanted to flirt with her. Jo says she's delivering twins today and explains her dual specialty. She asks about his profession. He works as a pilot. Sam assures her he's a simple guy but the videos she's seen of his wingsuiting on social media tell her another story. Mika comes in and Jo heads back to OB.

Lucas finds Ray as he signs the discharge form in the lobby. He convinces Ray to let him talk about the surgery during a walk outside.

Maxine arrives at her room, where she's surprised by her friends Norma Douglas and Lois. Jules says they can only stay for an hour. They brought Maxine's crochet bag. Maxine heads to the bathroom to brush her hair. She takes a fall in there and Jules finds her with a head lac.

Richard approaches Winston and Nick at the nurses' station. He asks Winston if it will be awkward to see Maggie so soon. It will but he's coming anyway since very few Black men have been nominated. He's paged away for a consult. Richard then tells Nick that the Award means something to people here. And if Winston can rise and see Maggie, Richard thinks Nick can handle facing Meredith. Nick doesn't believe in ranking medical contributions. Richard says his absence would be an insult to the hospital. He suggests Nick get off his moral high ground and do his job.

Bailey has joined Jules in Maxine's room. An X-ray shows a rib fracture, which will heal on its own. Bailey orders a head CT to make sure she had no brain injury. Maxine is no longer up for visitors. Jules promises to call Norma and Lois with updates.

Mika and Simone are working in the skills lab. Simone is double-checking wedding RSVP's while watching an old AAA surgery. Mika confirms that she's coming despite not having sent in the card. Lucas isn't coming. Mika hasn't thought about bringing a plus one and it's already this weekend. Simone reminds Mika they moved up the wedding because her moment of lucidity is shrinking. She does remember weddings. They have real conversation about the wedding. Taryn comes in to take a look at the new skills lab. She approaches Mika, who is mad that she projected her own trauma onto Mika's situation. She'll get over it but she needs to stay mad for a while. Taryn leaves and Simone comments they have chemistry. Mika is not a fan of rejection after since she got turned down at her promposal, the video of which went sorta viral at her high school. She doesn't make the first move anymore. Simone suggests she could bring Taryn to the wedding.

Ray tells Lucas about his job as an illustrator for graphic novels. Ray had a dream as a kid of his grandpa sitting down on his bed and telling him they would be together for his 30th birthday. It scared him and made adapt a healthy lifestyle. He just needs to get past 30. Lucas then draws the surgery for Ray to explain it. He says the procedure has been tested over and over and they can address its complications, but they cannot help him if the aneurysm ruptures at home.

As she's getting her scan, Maxine tells the doctors she can't remember getting dizzy. She just lost her balance. It might be a sign that her time has come and when a race horse breaks its legs, you put it down. Jules firmly tells her she's not a race horse. The scan reveals a subdural hematoma. Amelia has Benson book an OR for a decompression. Jules wants to scrub in but Bailey forbids her. She has proven to be too close to the patient so Bailey throws her off the case for good.

Lucas finds Teddy as she finishes up a call with Catherine. He got Ray to sign the consent forms. Teddy compliments his work.

Sam had Jo paged to his room. He needs help scratching his arm. Sam talks about his first cast, when he was 7. It's a miracle that he hasn't had a cast ever since, though he has had multiple surgeries. Jo is paged back to her twin delivery.

Winston is applauded by the surgical staff. Owen wonders where his award is for the all the people he saved. Amelia brings up her Parkinson's research. Bailey reminds them of her 12-person domino surgery, saving a boy with SCID, and basically curing post-op fistulas.

Benson wheels Maxine to the OR. She says if it wasn't for Jules, she would have her doctors just put her out of her misery. Benson knows Jules loves living with her. Maxine raised her son to be independent and now he lives in London and she hardly sees him anymore. Jules fills up that space in her life. It seems she never got that special motherly care as a child and Maxine is happy to provide it to her now. Benson says Jules is lucky to have her and holds her hand.

As they await Ray's scans, Lucas explains to Teddy and Simone how he convinced Ray. Nick comes in to thank Teddy for funding a practicum. He also confirms he won't be attending the dinner before the ceremony. Ray starts grunting in pain. Teddy rushes to him as the scans come up, revealing a rupture. Nick activates MTP and brings in the crash cart. Teddy decides to open up Ray right there on the CT table. Simone bags Ray and Lucas performs compressions while Nick and Teddy work to stop the bleeding.

In the OR, Amelia and Benson are performing the decompression.

Simone and Lucas switch places. Teddy asks for more lap pads as they try to evacuate all the blood. After a while, Ray continues to flatline. Nick defibrillates to no avail. He says they did everything they could.

Benson compliments Amelia's work as she closes up.

Teddy stops the CPR and calls time of death, 15:21. Lucas is left in shock.

Levi catches up with Richard and asks how things went with Taryn. Richard says she asked for thing they hadn't done before. Levi tells him that Helm is a proven asset and given the physician shortage, this should be a no-brainer. She should get whatever she wants.

Lucas makes the difficult phone call to Ray's mother to inform her about Ray's passing. Nick waits for him to finish. He then tells Lucas that with ADHD, you feel the losses deeply, so you have to take equal time to take in the wins. He fought for Ray and that matters. Lucas says Ray knew in his gut that he was dying. Lucas himself in his gut that he was different, his family are all doctors, yet he was never diagnosed. Nick says parents see kids through the lens of their own experience. His parents were too busy pushing him to think about why he wasn't living up to their expectations. Lucas hopes Nick wins the award. He feels like a failure now and if Nick wins, it would give him hope for himself. Nick says it doesn't mean anything.

Sam asks Mika how long a delivery usually takes. She tells him women don't like aggressive flirting. Sam tells her you never get anywhere without asking. Jo comes in and says it was a false alarm. Sam appreciates that she came back to be cheered up by her favorite patient. Jo claims she came to check on Mika. The Doppler signal on right leg sounds weak. Jo opens up the bandage and finds that the flap is congested due to a hematoma. Jo needs to decompress it. Sam refuses additional morphine. He wants to see this. Jo makes a cut and blood squirts out all over her and Mika. Sam grunts in pain as he watches.

Benson finds Jules in an on-call room and informs her the surgery went well. He brought her the flowers she had bought Maxine. Benson knows what it feels like to worry that your loved one won't make it. It's okay to have those feelings and he's here for her. She claims she is fine and says she needs to call Norma and Lois. Benson leaves, after which Jules starts crying.

Jo and Mika bandage up Sam's leg. Jo says what they did was not like swimming with sharks. Sam concludes that she went deep into his social media since that post is at least from three years ago. Mika comments she feels like a third wheel. Nurse Stacy comes in to inform Jo that her patient was moved to L&D. Sam tells Jo that she is his hero and he has her sign his cast. Once Jo's gone, Sam tells Mika that is what asking gets you.

Nick finds Richard, who says he cares about this program and this nomination will get it back to where it was. Nick wanted to ask about a seat on the plane. Richard will arrange that.

Levi meets Taryn at the bar. She gives him his usual burger order. Taryn announces that she gave notice. She is coming back but there is something she should tell him. Since she values resident wellness, she wanted to be in a position to help with that. So, Webber agreed to make her co-Chief Resident with him. Levi hugs her. Also, she got 8 weeks of vacation.

Mika enters the bar and intercepts Taryn as she's clearing a table. Mika brings up the wedding and says she has something uncomfortable to say. She likes Taryn and if she's interested, she would like for her to be her plus one as a date. Taryn then says she's about to be her boss but since the wedding pre-dates her employment, she's up for it.

Jo is checking Sam's Doppler signals, which are good. She delivered the babies. He asks her to scratch his back. Link comes in and says apparently Sam's covered. He heard about Jo's decompression. Link says he's heading home and asks Jo if she wants some food. Sam asks if they live together. Link says they do as Jo says they are roommates. Jo tells Link she's good and she'll see him later. Link leaves as Jo and Sam engage in some small talk.

Lucas arrives at home and tells Simone that he probably has ADHD. There is this symptom where you get stuck in your head and overthink and have trouble saying what you want to say. He asks her not to get married. Trey sees an old version of her but Lucas sees her for who she is. And he loves every single he sees. He walks off as tears well up in her eyes.

Sam was two days post-op on his surgical limb repairs. All doppler signals were strong and he had no signs of infection. On one of the checks, Mika noticed the pulse sounded weak, which Jo confirmed. She pulled back his bandage and said the flap was congested. He had a hematoma putting pressure on the flap, cutting off the blood supply. Jo opened his knee to decompress it in his room. It worked and he stabilized.

Maxine Anderson[]

Max, 81, was still in the hospital following her sepsis. Her delirium had resolved and she'd been off pressors for 24 hours, so she was moved out of the ICU into the step-down unit. When she arrived in her new room, she went to the bathroom to brush her hair, but slipped and fell, hitting her head. X-rays showed a fractured rib, for which she was told to rest. They ordered a CT for her head. The CT showed a brain bleed, so Amelia scheduled a decompression. Her surgery went well and she was moved to the ICU afterward.

Ray Sanchez[]

Ray, 29, had been diagnosed with an abdominal aortic aneurysm. He was very anxious about having surgery to clip it and left the hospital. He'd previously left without having the surgery on three other occasions in the two months since his diagnosis. Lucas talked to him and explained exactly how the procedure worked and he agreed to go through with it. However, while has having a pre-op CT, the aneurysm ruptured. They instituted massive transfusion protocol and opened his abdomen in the CT scan room, but they were unable to resuscitate him and he was pronounced dead at 15:21.
