- Over a period of six months, interim Chief of Surgery Meredith works to rebuild the program and modernize the OR's. She assembles a new group of interns, all of whom are "diamonds in the rough" who either deserve a second chance or a first chance that they had previously missed out on due a variety of circumstances. When Zola starts suffering from panic attacks, she is a formally diagnosed with exceptional intelligence, prompting Meredith to seek out an appropriate school. She finds one in Boston and decides to relocate with her family after being invited by Jackson Avery to use Foundation funding to research a cure for Alzheimer's disease.
- Bailey rejoins Grey Sloan's surgical staff as an attending general surgeon and dedicates herself to women's reproductive health, opening up the Elena Bailey Memorial Clinic for Reproductive Health with visits from Addison. Their efforts in the field make them the target of the anti-abortion extremists and they end up being doxed.
- Teddy and Owen's marriage comes under pressure after their savings are drained to keep Owen out of jail. They reconcile after Bailey intervenes and Teddy is appointed Chief of Surgery after Meredith's departure.
- Maggie and Winston's marriage crumbles as professional dissatisfaction - and a resulting idea for a career change - from Winston confronts him with Maggie's ambitious side. A publication on an innovative surgery leads her to accept an offer to lead high-end research at the Heart Center of Chicago. Having finally found his footing at Grey Sloan and not wanting to be a trailing spouse, Winston decides to stay.
- Amelia maintains a long-distance relationship with Kai until they announce that they have taken a dream job in London, ending the relationship. Having lost them and both her sisters, Amelia starts spiraling.
- After dabbling in OB/GYN, Levi returns to the surgical residency program and is appointed Chief Resident.
- Nick moves to Seattle at Meredith's request and becomes the hospital's residency director. Communication issues over her decision to move to Boston once again cause friction between the two and he decides not to follow her to Boston.

- The season is notable for multiple changes to the main cast as well as being the season with the largest main cast featuring 17 series regulars:
- Ellen Pompeo signed a new deal for the season with another salary increase, also upgrading from co-executive producer to executive producer. The contract renewal talks went over smoothly compared to the renewal talks for season 18, which lasted until May. It was later revealed that the deal included a reduced involvement in the series with her only appearing in eight episodes for the season yet she will continue to narrate the rest of the episodes.[source?]
- Chandra Wilson and James Pickens, Jr. had previously signed a multi-year deal ahead of season 18.[source?]
- Kevin McKidd, Kim Raver, and Camilla Luddington all had signed three-year contracts ahead of season 17, meaning their contracts are up at the end of this season.[source?]
- Kelly McCreary's contract was up at the end of season 18 but it was renewed for an unspecified length of time.[source?]
- The season adds five new series regulars (Niko Terho, Harry Shum Jr., Adelaide Kane, Alexis Floyd, and Midori Francis), the highest number of new series regulars to date. All five portray new interns at the hospital, meaning the new intern class features two men and three women, just like the original group of interns that started out at the beginning of the series.
- The first table read of the season took place on July 28th, 2022.[1]
- Filming for the season started on August 3, 2022. The day before, there was a photo shoot for cast photos of (at least) the five new series regulars.[2]
