The show was officially renewed for a twentieth season on March 24, 2023 for the 2023-24 broadcast season. The 2023 Writers Guild of America and 2023 SAG-AFTRA strikes caused a delay in production. While writing for the season resumed in early October,[1] the actors did not return to work until November 15, 2023.[2] The season premiered March 14, 2024 and aired on Thursdays at 9 PM.

- Meg Marinis served as the showrunner for the season, taking over from Krista Vernoff, who left the show at the end of the nineteenth season.[3]
- This is the first season since season 11 not to feature Kelly McCreary as Maggie Pierce as a series regular. She did, however, make a guest appearance.
- The table read for the first episode took place on November 15, 2023.[2]
- Filming for the season wrapped on April 10, 2024.[4]
- For the first time, the season does not feature an episode that shares the same number as the season itself—a recurring theme previously seen with Episode 5 in Season 5, Episode 16 in Season 16, and so forth. This break in pattern distinguishes Season 20 as the only season without this numerical alignment
- This is the first and only season in which no series regular is absent in any episode. However, Ellen Pompeo made 5 voice only appearances.
- This is the first season since his introduction in season 6 where Jesse Williams does not appear as Jackson Avery.
- Meredith narrates all of the voiceovers this season, just like happened in season one and eighteen.
