The show was officially renewed for a twenty-first season on April 2, 2024 for the 2024-25 broadcast season. It premiered on September 26, 2024 and aired on Thursday at 10/9c PM. The season consists of 18 episodes.

Plots[]
- Meredith and Catherine keep arguing about the Alzheimer's research, and Meredith prepares for an inevitable legal battle against her until she discovers that Catherine's cancer is progressing. Meredith is forced to treat her and hide the news from Richard.
- Jules and Mika begin dating, but when Mika discovers that her sister Chloe has cancer, she ditches Jules to support her. However, when they are in a car accident, Chloe dies, leading Mika to decide to leave the hospital, leaving Jules heartbroken.
- Jo deals with her frustrations and fears upon discovering she is pregnant with twins, which strains her relationship with Link.
- Kwan attempts to restart his relationship with his amnesiac ex-fiancée, Molly Tran. However, Molly decides to undergo experimental surgery to try to recover her lost memories. When it fails, Kwan realizes she is only clinging to him out of desperation.
- Levi begins dating James Morgan and is offered a research position in Texas, which he decides to accept.
- Jo and Lucas get in the middle of a robbery at a convenience store.
- Lucas asks Catherine to reconsider his remediation.
- Lucas and Simone's relationship begins to fracture due constant disagreements and Simone's fear of going too fast.
- Ben returns to the hospital as a probationary resident, but struggles to adjust and follow the rules, causing conflict with Teddy and Bailey.
- Teddy and Owen decide to open their marriage.
- Link and Jo get married.
- Jules asks Winston to be her mentor and the two of them work wonderfully together until he starts developing feelings for her and pushes her away.
- A patient's worried mother holds Amelia, Simone, and Lucas hostage and forces them to operate on her daughter. This leads to an explosion when they learn the gas tank she used had filled the room with gas.

- The show was officially renewed for season 21 on April 2, 2024.[1]
- On May 13, 2024, it was announced that Jake Borelli would be leaving the series after season 20 with negotiations taking place for him to return for season 21 for a few episodes to wrap up Levi's storyline.[2]
- On May 15, 2024, it was announced that Midori Francis is leaving the series as well, with negotiations taking place for her to return for a few episodes in season 21 to wrap up Mika's storyline.[3]
- Furthermore, it was announced that as part of a cost-cutting move, several of the veteran series regular would face a reduction of their episodic guarantee (the number of episodic fees per season each series regular is entitled to per their contract), likely resulting in less appearances by said regulars throughout the season.[2]
- Meredith narrates all of the voiceovers this season, just like happened in season one, eighteen and twenty.
- Filming for the season presumably started on July 24, 2024. James Pickens, Jr. and Anthony Hill filmed together that day, as did Camilla Luddington and Chris Carmack. Jake Borelli shot his first scenes for the season that day as well, although other actors may have shot scenes on the days prior.[4][5][6]
- Filming for the season wrapped on March 27, 2025, twenty years to the day after the first episode premiered.[7]
- On August 19, 2024, they were filming I Can See Clearly Now.[8]
- The table read for This One's for the Girls took place on August 20, 2024.[9]
- On January 9, 2025, production shut down due to the L.A. wildfires. It resumed on January 14.[10]
- This is the first season since her introduction in Season 10 that does not feature Kelly McCreary as Maggie Pierce.
