As Maggie is packing up her stuff in her apartment, a wedding photo of her and Winston on the nightstand catches her eye.
Winston jogs past Richard in the hospital parking lot. Richard is confused to see him by himself.
Kai arrives at Amelia's apartment. She took the whole day off. They start kissing.
Simone and Lucas arrive at the hospital. Simone removes another one of Bailey's doxing flyers from a car.
Ben walks Bailey to the entrance. He thinks she should change her cell phone number and email address but she refuses to let the doxers win. Ben thinks it will only get worse and suggests they move to a rental home for six months. Bailey insists it will blow over. When she receives yet another call from an unknown number, she decides to have an intern babysit her phone.
Mika is woken up by the arrival of the other interns. She did a late shift at Joe's and slept in the locker room. Simone and Lucas have a cake tasting tonight. Jules regrets that it's Maggie's last day today. The interns are surprised when Mika shares that apparently, Winston is staying. Jules thinks marriages are distracting from realizing one's full individual potential. Benson points out that Mika still looks bewildered before she can head out.
A giggly and smiling Jo and Link get on the elevator with Teddy and Owen. Jo used an in with a popular pizza place's manager to get her and Link a reservation. Once they get off, Teddy comments on their flirting, deeming it messy. Owen recalls them being like that but Teddy is not having it.
Mika and Lucas arrive in the ER. He comments on her smelling like booze but she quips that he's being the man of honor for someone he loves. Paramedics bring in William Hudson, with a left upper extremity deformity and abdominal contusion. As they move him to a trauma room, paramedics reveal he tried to mail himself to his girlfriend in Florida for her birthday and fell off the conveyor belt. Mika goes to page Owen.
Viv Soares and Nola Marques intercept Jules and Simone in the lobby. They insist on speaking to Dr. Maggie Pierce, whom they drove 400 miles for. Nola has a giant thymoma that is crushing her heart and they won't leave until they've spoken with Maggie.
Richard finds Maggie packing up her things in the attendings' lounge. She acknowledges that it is all happening fast for her but it's a huge opportunity. Richard thought Grey Sloan meant more to her than a stepping stone. In his day, you accepted a job after your fellowship and stayed there until retirement. Loyalty mattered. Maggie asks if she should not pursue research that would save thousands of lives, out of loyalty. Richard points out she has started doing that here with Winston. Maggie starts saying that she will not apologize but is interrupted by Viv screaming through the intercom to see Dr. Margaret Pierce at the nurses' station. Maggie tells Richard they'll talk later and leaves.
A crowd has gathered around Viv, who has gotten a hold of the phone at the nurses' station. Maggie arrives. Jules says they called security. Viv is excited to meet Maggie and shows her Nola's scans on her phone. Maggie has the interns arrange a room for Nola so she can do a proper consult, though she warns Viv not to ever pull a stunt like this again.
Levi is examining Grayson Friedman, who has abdominal pain. Richard arrives and greets the parents. Benson presents the case. Richard diagnosed Grayson with ulcerative colitis when he was 8 and so far, they have managed his case with meds. This morning, he collapsed due to pain. Grayson is afraid he'll need surgery, meaning an ostomy, but Richard tells him they'll find out what's going on first.
Outside the room, he orders labs, meds, and scans. Levi then asks him to talk about fellowships. Richard is dismayed to hear he hasn't applied yet since he submitted recommendations for most candidates months ago. Richard tells him to get his act together. Benson pretends he wasn't listening.
Viv is impressed that Nola is being examined by the Maggie Pierce. Simone presents the case. Nola has a thymoma that is compressing her heart and vasculature, leading to SVC syndrome. Viv and Nola have been friends since elementary school. They make up a band called Double Helix but they haven't performed since Nola's diagnosis. Viv dragged her to every heart surgeon in the Northwest but they all deemed it too risky. Maggie wants new scans. Jules and Simone leave to arrange the imaging.
In the trauma room, William tells his doctors he tried to pull off a grand romantic gesture. Owen likens it to his showing up in Germany. Link orders films to assess a possible fracture.
As they walk off, Teddy says the lesson here is that romance can be fatal. Owen says it can also save lives. Teddy thinks he's just trying to prove her wrong since he missed the fact that Jo and Link were an attending. Link counters that they are just friends. Teddy adds they slept together, much to Link's surprise, and asks to have her paged when the scans come in.
As they await Grayson's scans, Benson asks Levi how he got to his fourth year without a clear direction. Levi says there was the pandemic and the residency program being shut down. Benson tracks each subspeciality in a spreadsheet and monitor trends in each area. Grayson is growing uneasy. Levi tells him to sing or hum a song. Grayson starts chanting a Hebrew song. He has been studying for his bar mitzvah. The scans come up, revealing severe acute colitis.
Kai is cooking at Amelia's appartment. They suggest a walk after they've eaten. Amelia realizes there will be no more Sunday dinners. The leftovers from Maggie's cooking always get her to Wednesdays. Amelia has a history of spiraling when people leave her, which entails pulling away from loved ones, starting fights, lack of sleep, and spilling your worst instincts to people who flew in to care for her. Kai acknowledges it's a big change. Amelia will do her best not to be sad while Kai's there.
Bailey arrives in the ER and says she has a job for Mika. She entrusts Mika with her phone. She can only answer calls from within the hospital or her husband, Ben Warren. Lucas jokes she might get to babysit Bailey's tablet next. An anonymous call comes in.
Levi and Benson are with Grayson, whose bar mitzvah is 36 hours away, when Richard comes storming into the room. He knows Grayson hasn't been taking his medication due to his lab results. Grayson admits to it. The meds make him tired and he once fainted for everyone to see. His eyes started burning and he got a gross rash that Eloise Schurmann saw. Richard cuts him off and says he needs surgery. Grayson thinks he can make it through his bar mitzvah without surgery but Richard disagrees. Benson says the inflammation would spread further and he could die without surgery. Richard says he'll talk with Grayson's parents since he clearly is not mature enough to understand the severity of the situation.
Owen wants to go over William's films with Link, who keeps getting distracted by Jo's texts. It's a simple mildly displaced humeral fracture, which Link will reduce once Owen and Teddy are done with the ex-lap for the splenic laceration. Link keeps laughing at his phone. He thinks Owen thinks he should tell Jo but they have build a home together as friends, which could all fall apart if Jo doesn't feel the same way. Owen has no opinion on the matter and his last relationship advice didn't turn out well. Mika and Lucas pass by and go to prep William for surgery.
Jules and Simone are doing pre-op work on Nola. Jules's parents toured with a band briefly. Viv is Nola's only emergency contact. Jules comments you don't need a spouse when you have a bandmate. Simone says they're going to talk to dr. Pierce.
Outside the room, Simone asks Jules why she's hating on marriage when Simone is about to get married. Jules doesn't care. Simone insists it's not a distraction since she has been focused on becoming a surgeon since she was 8 years old. Jules wonders who she's trying to convince. Viv calls for help. Nola started gasping and lost consciousness. The interns start oxygen and call for Maggie.
Maggie arrives and is informed Nola has JVD, hypotension, and muffled heart sounds, indicating a cardiac tamponade. Maggie guides Simone through performing the pericardiocentesis, which she pulls off flawlessly. Her vitals return to normal. Maggie says they don't have much time before the fluid builds up again. She has Winston paged and Nola moved to the CCU.
As Mika and Lucas wheel William to the OR, Bailey's phone keeps buzzing. Mika decides to answer and cusses out the caller, only to find out it's the optometrist calling about Tuck's glasses.
Grayson is playing Minecraft to distract himself. He hopes Richard will still want to be his doctor after disappointing him. Levi is sure he will; he disappointed Richard, too. Grayson says his classmates call him defect since he's always in the nurse's office taking his meds. Levi understands what's it's like wanting to be normal but he stopped doing that and ended up liking himself a lot more. Grayson says he's ready for the surgery.
Winston meets Maggie in the imaging room. She pushed her flight to tomorrow. She informs him about the case. He notices the tumor is heart-shaped, which she doesn't care about. They discuss multiple approaches. She decides to use a bovine pericardial patch for the reconstruction, which would allow them to have clear margins. They decide to operate just the two of them, no interns, since there is no margin for error.
Amelia is crying in Kai's arms as they lie in bed. Amelia is so glad that they are there, even if she's scaring them. Kai says it takes a lot for them to scare. Amelia says this is the first relationship in which she can be completely herself, the queen of spirals. Kai says that is just a small part of who she is. Amelia fears they are underestimating her ability to fall off the deep end. Kai finds the Amelia that they know to be inspiring.
In the gallery, Simone starts listing married surgeons with stellar careers. Jules moves a couple of seats away from her.
Down in the OR, Maggie warns Winston to be cautious of the friable tissue. They remove the tumoral mass. Afterward, Nola's blood pressure soars but Maggie and Winston wait it out. It's a pseudo-tamponade effect, Winston explains. Nola's heart is no longer used to having so much space but now it is alone and happy. Maggie says it's time to start the reconstruction. They start Nola on bypass.
Teddy, Owen, and Link inform William that his splenectomy and humeral fracture repair went well. Teddy leaves the room to update his chart. William says Rose dumped him after she heard about what he did. He fell for her the second he saw her waiting in line for the Space Mountain and thought she was the love of his life. Owen says he and his wife were friends for years. They met in the Army and even though it took years, he knew from the moment he first saw her that he wanted her in his life. Their timing was never quite right for years but their friendship outlasted them all, even when she moved to another country. One day, Owen realized she was the person that he wanted to talk to about everything. He then got a plane and told her how he felt. If William knows he wants Rose in his life, he should try again. Teddy, Link, Lucas, and Mika listen closely to his story. William then realizes Rose isn't it. Link excuses himself.
Richard removes nearly Grayson's entire colon. Richard calls him foolish to the point of stupid since the meds worked wonders for years. Levi says at Grayson's age, he had a hard time, too, so he knows that Grayson is beating himself up over this. None of them heard Grayson tell them that his meds made him feel terrible on top of working his butt off for an entire year on becoming a man. So instead of chastising him, they should try listening to what the other person is telling them they need. That is what being a good doctor or mentor means.
Maggie gets the patch ready but since there is not back wall left, Winston is not sure the patch will work. He suggests they access the jugulars and use reverse cryoartery and drain into the right atrium. Maggie agrees with his pitch. They start working in sync on a jugular vein each. They craft a vascular reconstruction and insert it without a hitch. After they've closed up the incision, the crowd in the gallery applauds. They bask in the glory for a minute.
Mika and Lucas are annoyed due to the constant buzzing of Bailey's phone. Lucas ends up throwing it into a trash can, where it continues to buzz. Mika takes the phone out of the trash just as Bailey appears. Mika suggests turning the phone off but Bailey says she can't do that as a wife, mother, and high-level professional. She takes back her phone and is shocked to see she has 248 missed calls. The seriousness of the situation dawns on her.
Viv sees Maggie and Winston approach the waiting room. Based on their faces, she fears that Nola has died but Maggie happily tells her that the surgery went well. Winston says they have a lot going, which explains their faces. Viv knew Maggie was the right person for the job. Maggie could not have done it without Winston. Viv hugs them both.
Grayson wakes up in his room as Richard is checking on his ostomy. His parents are out to get something to eat. Grayson didn't mean to upset them or Richard. Richard remembers the moment they diagnosed him. He was worried about missing his end-of-class party back then. Richard looks forward to seeing what's next for him and it's frustrating when he makes a decision that puts his future at risk. He apologizes if that came off as anger. Richard says they'll get him started on a new kind of medication. Richard makes him promise to tell him if the new drugs causes him any problems so they can find a solution together.
Owen and Teddy ride the elevator. She brings up his speech and says he went for his two favorite things: proving her wrong and getting laid. Owen was just trying to make the patient feel better and asks if it worked. Teddy says his mother is watching the kids for the night.
Simone finds Jules in the skills lab and tells her about Dr. Mary Edwards Walker, the first female surgeon in the US, who is married and started a practice with her husband. Simone says the wedding is a pain in her ass. The dress shop is harassing her to come get measured but she's at the hospital all the time. She can't ask Lucas to measure her and her sister is not talking to her. Jules then starts taking her measurements.
At home, Link is struggling as he pratices his declaration of love to Jo. He thinks he should have written a song. He answers a call from Jo, who is crying. The daycare thinks there is a problem with Luna's hearing and she needs to get checked out. She's coming straight home with Luna. After she hangs up, he blows out the candles and throws the flowers he had set up all throughout the loft in a trash bag.
Ben meets up with Bailey and Pru in the lobby. Bailey says they are not going home. She got them a rental. An anonymous number sent her a picture of Tuck in school. She changed her number and her e-mail as well.
Mika wakes up to the other interns doing the cake tasting together. Lucas throws her a sample.
Winston catches up with Maggie and tells her it was a hell of a way to go out. Maggie says wouldn't have figured it out without him, like so many other things in the past. He challenges her and pushes her to be better. He inspires her to never give in. She was also in awe of the way he accessed that jugular vein.
At home, the two of them get undressed in between the moving boxes. They fall onto the bed and proceed to have sex.
Kai and Amelia are naked in bed together. Amelia asks Kai not to go back to Minnesota. Kai says they're not. The team has asked them to set up and run a lab in London with triple the size and staff. They wanted to tell Amelia earlier but she was already so sad. Kai already accepted the offer. Amelia starts crying.
