Mika waits outside the hospital parking lot.
Not feeling nauseous for the first time in weeks, Jo joins Link in the bathroom for a shower. She reminds him they have an OB appointment after work today.
Bailey updates Ben on the new EMR system. It feels like coming home to him so he's not nervous. Richard and Winston come up. Ben remembers Winston from his wedding to Maggie, ignoring Bailey's attempts to shut him up. Richard talks about the party that Catherine wants to throw for her biopsies coming back negative. As they walk off, Bailey informs Ben of Winston's divorce.
Simone, Benson, and Lucas are almost late on Bailey's first day back as residency director. Jules is running late as well due to a fist fight on the bus. They come across Mika, whose sister arrives with her rideshare driver moments later. She lost her phone at the airport. Mika introduces Chloe to her colleagues. Chloe blurts out she has cancer. The interns head inside to meet Bailey while Mika hugs her sister. She says they'll figure this out with Dr. Webber.
Taryn meets up with Levi outside the hospital. He hands her the overnight roster list with patients to follow up on. It's Taryn's first day back after her trip in Paris. James comes up and Levi introduces him to Taryn. James worked overnight as well. Levi suggests they get coffee together, to which James agrees.
The interns discuss Ben's joining the program while he's welcomed back by former colleagues. Jules reminds everyone he's here on a trial basis until the ACGME approves him to stay. Benson thinks of "Mr. Dr. Bailey" as his competition. Ben comes up and says it's Ben Warren, actually. Bailey arrives and tells the interns that Ben will not be getting special treatments. There are four months left for their intern year and she wants them sprinting to the finish line. Taryn hands out the assignments.
Teddy is surprised to see Owen in front of the OR board. He was called in around 4 AM. The kids are with his mother. Teddy had bagels dropped off at the house for a surprise since he's been on the morning routine so often lately. She has back-to-back meetings and she has to finish the quarterly budget report. Owen tells her to pace herself. She tells him that the job is very different now from when he did it 10 years ago. Teddy leaves to fire an anesthesiologist.
Taryn arrives in the loading dock with Lucas and Jules as medical transport arrives with a truckload of cadavers for Ander-Simmons's med school anatomy lab. They will help scan the cadavers so the students have images to help them determine how their cadaver died during their dissection. Taryn leaves them to it.
Winston and Simone round on Darren Riley, 55, a high school band director with pulmonary fibrosis. He was admitted with multi-drug resistent pneumonia. He's currently on 7 liters of O2 and IV antibiotics. Darren recognizes music in Simone's voice. She talks about marching as a trombone player in high school. Darren's sats are low. Winston tells her to move Darren to high-flow O2 and get a CT. Winston quietly instructs Simone to keep a close eye on him given his dire status.
Richard is meeting with Mika and Chloe. He reviewed Chloe's scans, which show stage 3B rectal cancer rather than 2A. They will have to start with chemo and radiation and depending on the tumor's response, surgery may become a possibility. Chloe asks about the side effects. Richard lists the most common ones, including impaired ovarian function. They will set up a meeting with her OB/GYN to discuss options regarding fertility. Mika starts experiencing anxiety as Chloe and Richard continue talking.
After getting coffee, James and Levi discuss the former's travels. He did the whole global-nomad thing before divinity school. Levi has never really travelled. James grew up in Wyoming with two devout Episcopal parents. It was complicated. Coming out wasn't that hard as an only child, but he always felt pressure to be their everything. He needed space, moved out, and then they died. Through the guilt and grief, he found his faith again, which led to his becoming a chaplain. Levi says you sometimes have to help yourself in order to help others. He learned that phrase from his mother, which is a whole other conversation. James suggests they get lunch.
Ben arrives in the busy OR. He tells Owen he's still on a trial basis but he asks Owen to treat him like any other resident. Owen suggests he warm with some sutures and instructs Benson to help him out. They meet with Judith Dunn, who has a cut on her thigh. While Ben struggles to find the right supplies, Judith talks about how six-inch stilettos and tequila don't mix. She is proud to show off her Brazilian butt lift. Upon seeing the cut, Ben admits he grabbed the wrong sutures. Benson already has the correct ones ready. Judith complains of hip pain. Ben says they'll get an X-ray and labs to be sure. He asks Benson to take care of the labs.
It takes a lot of effort for Lucas and Jules to get the bodies into the scanner. They have to flip the body around.
Simone hovers outside Darren's room as Bailey comes up. Before she starts rounds, Bailey asks Simone to run to the pit first to discreetly observe how the patients and doctors there are doing. Simone understands she's asking to check up on Ben for her and says she'll report back after.
Jo is meeting with Mika and Chloe. They typically recommend hormone replacement therapy to help with fertility. To increase her chances, she can have her eggs retrieved and frozen. Mika says that'll take at least a month, if all goes well, during which time the cancer could advance. Mika decides they need to find some other way.
Link arrives in the ER to look at Judith's X-ray but Ben forgot to order the AP view. Since hip joint effusions are difficult to see on X-rays, Benson suggests a CT. Link agrees. Benson offers to show Ben how to order it but Ben is confident he can manage.
Mika runs into Jules. Mika briefs her on Chloe's condition. She is being seen by the Chief of Oncology. They have to start treatments right away, meaning they have no way to preserve her fertility. Jules inquires if Chloe wants kids. Mika replies she's only 22. She's the eighth out of eight sisters and now that she finally got out of her sisters' shadows, this happens. Jules tries to see the bright side and comments she'll still be alive even if she can't have kids. Mika says there is no bright side here and rants about how Jules can't understand this feeling given her terrible connection with her own family.
At Joe's, Levi and James have a heated discussion about Star Wars. They agree to head back to James's place, where they proceed to have sex.
Lucas and Simone share a quick kiss in the stairwell as she heads back to the cardio floor. Simone asks if he were asked to evaluate a colleague's performance and they made an inconsequential mistake, if he would report it. Lucas wouldn't, which settles her dilemma. He says his project with Taryn is deathly boring.
Richard finds Mika in the research library. Chloe went out to call their parents. Richard sees Mika is reading up on fertility studies in colorectal cancer patients. Richard knows surgeons like to think they can solve everyone's problems. Mika has found that Jo appears to be right. Over the years, Richard has learned that every cancer treatment is gruelling. Cancer is universal in the way that all patients ask the same questions, have the same concerns. This gives Mika the idea to look for ways to protect fertility in other cancers and see if it might be relevant for colorectal cancer too.
Ben meets up with Jo, who is excited to see him. He has her help him cancel a wrong medication order. Ben has forgotten how first days are always rough. Jo is sure that he'll be fine. She's still a constant mess after her own career switch. He asks what's new in her life. As she goes to tell him about the pregnancy, Mika comes up with studies and asks Jo for her help.
Owen catches up with Teddy to give her a turkey burger for lunch. He brings up how he doesn't understand why this patient of his got a Brazilian butt lift. Teddy breaks it down for him. Unlike men, women do not live in a world where they get rewarded for doing a great job. Even if you work harder and are smarter, the person with the more desirable body most always gets ahead because those decisions are made by men. Instead of judging the woman for changing her body, they should think about changing the culture with wrong priorities.
Benson and Ben help Judith get settled for her CT. She misses her booty pillow. Judith says her family gave her grief for getting the surgery. For 30 years, no one blinked an eye at her. Overnight, she became the star. She never felt more empowered in her own skin. As the scan starts, Benson receives Judith's lab results. Her white blood cell count is severely elevated. The scans reveal extensive signs of infection to the muscle along the fascial planes. He rushes to check on Judith. He removes the blanket, revealing necrotizing fasciitis on her leg. They book an OR right away.
As they cuddle and kiss in bed, Levi admires James's painting of a sunrise. He bought it in a café in Italy. He travelled there during a particularly rough time in his life and since the painting lifted him up, he bought it from the owner. He talks about feeling a strong connection as he gazes into Levi's eyes. They start making out again.
Bailey summons Simone from Darren's room after she did another ABG. Simone tells her everything appeared to be going smoothly. Bailey wants to make sure that Ben is treated equally to the other residents but her presence in the ER may take away objectivity. Simone says she can spare another five minutes.
Richard and Jo present Chloe with Mika's plan to move her ovaries into her abdomen so they are out of the field of radiation. It has been used before for patients with cervical cancer. There is a 50-80% success rate. She also won't need hormone replacement therapy. Chloe wants to have the surgery today so she can start treatment sooner and finish her last semester before her scholarship runs out. Mika wants to call their parents first. Mika clarifies she won't be allowed in the OR as her family. However, she will be as close as she can be.
Lucas is snacking while he and Jules wait for the scan to finish. Lucas receives word that Chloe is getting surgery. Lucas asks Jules what is going on between them. She says nothing. Mika is not texting her after accusing her of having no empathy.
Owen and Link meet Benson and Ben as they rush Judith up to the OR. They won't know how far the infection has spread until they get her to the OR. The bacteria must have gotten in through the cut on her thigh. Judith loses consciousness.
Winston enters Darren's room. Darren is crashing. Winston calls for Simone, who is not there. Winston moves to intubate and calls in Taryn for assistance. After intubation, Darren's sats do not improve. Winston decides to put him on ECMO.
Simone rushes into the room as Taryn and Winston are putting Darren on ECMO. She explains she was down in the ER but Winston is not interested. He instructs her to wait outside.
In the OR, the surgeons work to remove the infected muscle tissue from Judith's butt. Owen has Ben help out and compliments his move. Benson offers to help, too, but Link instructs him to keep up with the suction. Ben then has Benson assist him. They finally reach healthy tissue.
Lucas and Jules start scanning the last body. They talk about their med school cadavers. Having never met her grandmother, Jules realized her cadaver may be her grandmother. She hoped her grandmother never felt alone and had people who appreciated her. Lucas never thought about cadavers that way. Jules says she has something to do and leaves.
Bailey finds Simone in the CCU and asks for another update just as Winston leaves Darren's room. Darren is stable but Winston is not sure he'll ever breathe on his own again. Winston inquires why she was down in the ER. Winston continues to berate her until Bailey steps in and asks for a word in private.
A nervous Mika watches from the gallery as Richard and Jo help to get Chloe settled on the OR table. Just as she's about to be put under, Jules comes in and asks for permission to sit by Chloe's head. Jules holds Chloe's hand, which calms Mika down.
In the lounge, Bailey tells Winston she asked Simone to check up on the patient in the ER. Winston says she should have known to say no given Darren's condition. Bailey says it's hard to say no to your boss. She gives an evasive answer when Winston asks her why she was checking up on patients when she's not working in the OR today. Winston figures it out. As the guy who married and divorced his boss, he tells her to stay out of her husband's way at work.
Jo meets Link in the lobby. She forgot about her OB appointment and DeLuca had to leave. Jo tells Link to come with her.
Benson, Ben, and Owen check on Judith. They removed all of the infected tissue. They'll keep her for a few days to wash out the wound and make sure there is no more necrotic tissue. Owen is paged to the ER and tells Benson to come with. Ben sits down. He knows Judith feels like she's moving back into an old version of herself now. Everything she has learned and experienced is coming with her as she continues to move forward. The confidence she built for herself is still there. All is not lost. Down the road, she can still decide for reconstruction. He knows a guy in Boston.
Jules meets Mika outside Chloe's room. The procedure went well. Chloe will get to go home soon. Mika apologizes for earlier. It meant a lot to her that Jules sat with Chloe. Jules leaves as Richard arrives. Mika thanks him for everything. It's not easy watching someone you love fight cancer. Mika has to remember to take care of herself, too. Mika then enters the room to sit with Chloe.
Taryn finds Lucas assisting the medical transportation guys to load the cadavers back onto the truck. He scolds one of them for not handling the bodies with the proper respect.
Owen meets Teddy in her office. Owen wanted to grab a bite together first. They haven't had a conversation that wasn't about work or the kids in three weeks. Teddy is sure they'll get back on track eventually. Owen feels disconnected. Teddy feels te same.
Bailey finds Ben on a bench outside the hospital. Bailey wonders if her being his boss is a mistake. Ben is confident they'll figure it out as the professionals they are. He promises to let her know if it ever gets uncomfortable for her. They head home so he can get some sleep. She welcomes him back.
Chloe wakes up with Mika by her side. Mika tells her to rest. Chloe thanks her for everything she did as her sister. She then falls asleep again. The other interns come in with snacks, flowers, and balloons. They all took over her shifts for the next week so she can spend time with Chloe. Mika thanks them all.
Levi is eating ice cream as he looks around James's apartment while James is sleeping. Levi comes across a wedding album of James and another man.
Link is doing an ultrasound on Jo. She guides him to the right place. They are shocked to find out that there are two babies.

She found some cases of them surgically relocating ovaries higher in the abdomen to get them out of the range of the radiation, which would both preserve her future fertility and prevent her from needing hormone replacement therapy in the future. Her surgery was successful and she woke up asking for a donut.

Ben had Link look at the x-ray, but it didn't have the right view and he ordered a CT to give better visualization. As the CT came up, her labs came back and showed a high WBC count. The CT also showed signs of infection in her hip. They realized it was necrotizing fasciitis. They rushed her into surgery to remove the infection. On the way to surgery, she crashed. During her surgery, her Brazilian butt lift was flattened. She was awake after surgery and told they'd removed all the infection and would keep her in the ICU for a few days.
