Bailey struggles to get Pru to brush her teeth, resorting to singing "The Lion Sleeps Tonight" to get her to focus.
Simone is FaceTiming Trey as they discuss venues for the wedding. He knows she wants to do her research to find the perfect one. She moves to the common bathroom since she's out of toothpaste. Mika is in the shower and is horrified to find out there's no more hot water.
Teddy and Owen arrive at work with the kids. Owen is distracted when a rig arrives with a patient. Teddy tells him to save lives unsupervised and takes the kids to daycare as he gets to work.
As he gets out of the shower in the locker room, Lucas is berated by Mika and Simone for not having paid the gas bill. He sighs as the other interns get to work.
Bailey and Teddy meet up outside the hospital. Bailey is appreciative of Teddy's support of the clinic. There is an article about Addison's work with the PRT but Teddy finds that the online comments about Addison are vile. Addison meets up with them, looking more bewildered than usual. Addison left the PRT in hiding in Illinois until the press surrounding her work dies down. Addison claims she's doing fine. She's excited to get to work.
Bailey, Addison, Jo, and Carina arrive in the Elena Bailey Memorial Clinic for Reproductive Health. They welcome the inaugural class of out-of-state OB/GYN reproductive health and advocacy fellows. Their own residency programs no longer teach abortion care. Addison says their states are restricting their specialty, hence the fellowship to get them proper education. Addison has witnessed the heartbreak in their state. As the doctors get to work, Mika and Benson are feeling left out and decide to join the group.
Lucas presents the case of Jessica Hall, 22, who is suffering from idiopathic pulmonary fibrosis. She has been waiting for a transplant for years. Lucas is distracted by a notification on his phone. Today, Jessica will receive one lung from a directed donor, her boyfriend RJ. The transplant will prevent her from a further descent to heart failure. Jessica's mother is not a fan with RJ, who is now also late for the surgery. Lucas comments that surgeons are sometimes late, which is not appreciated by Maggie.
Owen is dismayed to find out that Winston, who ran the ER last week, has changed the categorization system to better fit cardio's. He loudly announces that he and his way of doing things are back. Nurse Marilu asks if Winston's habit of buying them lunch on Fridays will continue, which Owen says Winston can continue to do.
As Jo escorts a patient to a bed in the Clinic, Marcus Cole alerts Bailey to the presence of protesters outside who are trying to block the entrance. Bailey says they're usually peaceful, to which he replies, "Define peaceful." He shows her the screaming crowd outside with anti-abortion signs.
Owen has Jules prep for an incoming blunt chest trauma patient while he goes to meet the ambulance outside. The paramedic briefs Owen on the trauma. The patient, Ryan Jenkins, 24, was involved in an MVC. Ryan tries to talk but Owen tells him to save his strength.
Mika and Benson approach Andra, who is 34 weeks pregnant, thinking she came in for a medical abortion. It's actually her friend Marni Young who is seeking the abortion. She is six weeks pregnant and her lifelong best friend Andra is her support system. While Mika gears up for a quick physical, they hear a man chanting "This clinic kills!" outside.
Bailey and Addison finally get a hold of Teddy, who has been working on a budget issue with Richard. Addison says the protests intensified because of her presence, although she has no clue how they found out her whereabouts after switching flights to get to Seattle. Security recommends closing the clinic for the day. Addison points out that means denying care to dozens of women. Teddy decides to have security call for back-up and keep the clinic open.
Maggie finds that Lucas has failed to upload the scans. She thinks he's sluggish today. Nick has been busy picking up organs all over the country. He has no plans to go to Boston. Meredith texts him about once a week. Maggie gets all her updates from Zola. Nick says Meredith pretended not to hear him when he confessed his love for her. Maggie encourages him not to give up. Lucas arrives to update them on his progress, stating that a house thing delayed his uploading the scans. Maggie tells him not to get distracted. The scans come up.
Winston diagnoses multiple displaced broken ribs and a massive right-sided hemothorax on RJ. Owen places a chest tube as Winston says they need to rush him to the OR. Simone finds that Ryan's already in the system as Jessica Hall's lung donor. Winston has heard about the case from Maggie. As Ryan says they should take his lung, his sats drop.
Winston has just updated Maggie, Nick, Jessica, and her mother on RJ's accident. Owen is prepping him for surgery now since his chest sustained damage. Jessica's mother wonders how fast he was driving. Jessica gets upset with her. Maggie and Nick inform her that given his injuries, he won't be able to donate a lung today, or ever. Jessica doesn't care about that now. She throws her mother out of the room and begs the doctors to save Ryan.
Addison watches with a smile as Carina and Bailey teach the fellows.
Jo and Mika are with Marni. They hand her the mifeprestone. Marni lingers for a moment since she doesn't like swallowing pills. She is two years older than her mother was when she had her. She's still working through a lot so she's not ready to be a mother, but she will be an amazing aunt to Andra's baby. Marni declines Andra's abortion playlist. She takes the pill.
Lucas struggles to get someone on the phone to pay his gas bill. Maggie catches him in the act and orders him to come up with a plan to prevent heart failure for Jessica. He also has to arrange her discharge paperwork. Jessica will remain on the UNOS list but they'll send over her pre-op work to make sure her status hasn't changed. Once Maggie is gone, Amelia checks in on Lucas. He asks her if she knows the passcode to the gas-company account for the house. Amelia didn't handle utilities. He has to figure this one out for himself.
As they scrub in, Jules invites Simone to use the hot water at her house. And at least she won't want to have sex with Jules' roommate. Jules bluffs that she knows about Lucas. Simone falls for her trap and says she and Lucas just kissed, twice. Jules asks if the kiss was good. Simone says she's engaged. Trey is good, stable, and a responsible grownup. Jules derives that the kiss was amazing.
Jo briefs Bailey on a couple of clinic patients. Addison compliments Bailey on her choice of trainees. Bailey informs her that the silver lining was getting to learn from the great dr. Addison Montgomery. Bailey invites Addison to come teach new trainees ever month, if she can find the time between her work in Illinois and visiting her family back home. Before she can answer, a brick is thrown through the window, hitting Benson in the head.
Teddy instructs Bailey to keep everyone inside until there is enough security personnel for a safe evacuation. Richard joins Teddy in her office. He heard about the brick, which had "Montgomery murders" written on it. Teddy has yet to read all the crisis manuals since there has not been a moment without crisis during her tenure as Chief. Teddy puts on sneakers and heads out.
Mika is doing a neurological evaluation of Benson. He appears to be fine. Tia Marwood, a pregnant trainee, comes in and blames herself. She posted about getting to work with Addison on her social media, which gave away Addison's location. She's going to resign. For the first time in months, she woke up proud for joining the fight. Mika tells her not to resign; if she does, there is one less doctor willing to help.
All hospital staff is instructed to stay away from the clinic since the protests have turned violent. Owen and Winston are struggling to locate the source of bleeding in RJ's thorax. A lobe resection would leave him unable to donate so Winston suggests a wedge resection, preserving more lung tissue and allowing RJ to donate the other lung later. Clamping off the blood supply leaves Winston with only a few minutes to perform the repair but he's confident he can pull it off.
Jo informs Bailey and Addison that patients are getting restless and a trainee was crying in the bathroom. As an orderly sweeps up the glass, Bailey calls a team meeting. She tells everyone that the clinic is named after her mother, who taught her to help people who are struggling. As doctors advocating for women's health, they are all struggling, too, so they need to lift each other up. Bailey invites Addison to speak some words of encouragement. Addison hesitates. Marni then calls for help because Andra's water broke. While doctors rush to her aid, Addison retreats to the supply closet by herself.
Nick runs into Lucas and asks if he has Jessica's discharge papers on him. Lucas forgot to print them. Lucas says he has been wondering what he's even doing here ever since Meredith left. He keeps screwing up. He wants to believe that he's here on his own merit but he thinks he's only here because Meredith chose him due to his connection to Derek. Nick uses today's derailed transplant to point out that the things you worry about are almost never the things that really go wrong, while you could've never dreamt up the things that actually go wrong. Meredith is too good to have accepted him out of sentimentality, and so is he.
Bailey finds Addison crying in the closet. Addison needed a breath. Bailey says a usual protester brings a sign that reads "Please don't do this." She is peaceful yet it's still upsetting. Addison got a bulletproof vest. Clinics are being set on fire and staff is attacked with acid. Addison was doxxed. People broke into their garage and graffitied Jake's car. Jake and Henry are staying at Jake's sister's. Addison refuses to quit, because patients need this care, but she's exhausted and terrified. Bailey comforts her.
Marcus informs Teddy that he's setting up an incident command post with PD in the parking lot. Link joins Richard and Teddy as they walk down the hall. He offers to go down to the clinic, so he can also check on Jo. As Link and Richard try suggesting different approaches, Levi comes up to alert Teddy to a situation. Teddy makes them all shut up so she can think. She then sees that the lobby is flooded with clinic patients, who weren't allowed to go in by the protesters. Teddy has a plan.
A nurse checks out the protesters through the window as the doctors get ready to deliver Andra's baby. She is fully dilated. Andra comments that this is not how it was supposed to happen, with a chorus of protesters instead of her joyful delivery playlist. Bailey starts signing "The Lion Sleeps Tonight". The other staff joins in. Andra calms down.
Addison hears the singing from the closet.
Andra starts pushing.
RJ's pressure starts dropping. Owen and Winston up their pace but there are tears everywhere.
Jo has taken over for Carina but she finds that the baby's head is bobbing. Addison steps in and introduces herself to Andra. She explains that the baby's shoulder is stuck but drs. Bailey, Wilson, and DeLuca will assist so she can deliver the baby girl.
Teddy has turned a corner of the ER into the clinic, where Levi, Link, and Richard are seeing patients. Richard approvingly winks at Teddy.
Lucas hands Jessica her discharge paperwork. Jessica wants to stick around so she can see RJ after surgery.
Winston finishes just in time, removing the hilar clamp and allowing the pressure to climb back up. The next step is ventilating the lung to see if it inflates. If not, there is still a leak. Dr. Knox ventilates and luckily, the lung inflates.
With Marni's encouragement, Andra delivers a final push. Addison then presents her with a healthy baby girl. Addison and Bailey share a joyful look.
Lucas wheels Jessica's into RJ's room as Simone is checking on the chest tube output. Simone informs Jessica that dr. Ndugu only had to remove minimal lung tissue. If he recovers well, RJ might still be able to donate. Jessica doesn't care; she only needs him to be okay. She recalls their magical first kiss. The kiss changed her life in an instant. Simone leaves the room.
Teddy thanks Richard for his help with the clinic overflow patients. Richard tells her she never lost sight of the most important thing: keeping everyone safe today. She did well. She's an excellent choice of Chief.
Jo sees a bunch of worried texts from Link and lets him know she's okay. Marcus informs Bailey that PD is dispersing the crowd. They should be able to exit folks out the back soon. Jo gets Mika and Benson to help her organize people. Addison brings Marni and Andra a Montgomery mix to keep their blood sugar levels up. Marni is okay aside from some cramping. Andra says it was the hardest thing she has ever done in her life but it was all worth it. Addison smiles.
Nick meets Lucas in the imaging room. Lucas has come up with a treatment plan for Jessica. He arranged the necessary follow-up consults to get her to the transplant. Nick says it took him four times to get through the MCATs. The standardized questionnaire felt too absurd to him to measure one's ability to take care of people. He would always get up at one point or another and leave. It only worked out for him because someone helped him to figure it out. He instructs Lucas to sit down and has him focus on how to properly present a case.
In the locker room, Simone answers a call from Trey. Simone wants to get married next month so her grandmother can still come. It would mean a lot to her if they could do the ceremony in her grandmother's backyard. Trey is on board.
Down in the ER, Owen informs Winston that most of the protesters have been pushed back by PD. Owen tells Winston he pulled off an amazing save today. Patients would suffer if Winston went back to being a student in the OR. Winston nods and walks off for a consult. Teddy comes up and Owen tells her the job suits her.
Marcus escorts the doctors out of the clinic's back door. It's quiet on the street. Mika quietly encourages Tia to come clean. Tia then approaches Addison as she walks off. She thanks her for today. She skipped a babymoon to be here and she's glad that she did. Addison asks her how far along she is. Tia replies 28 weeks and then drops her stuff. As they kneel to collect her belongings off the pavement, Tia goes to tell her there is something she should know about today. Suddenly, a car starts speeding toward them. Bailey sees it coming and yells at them to get out of the way. Addison tries to pull Tia out of the car's path but Tia is hit, flying over the hood, while Addison herself is clipped, too. The doctors watch it play out in horror and then rush to their aid.

In order to give her more time to wait for a transplant, Lucas suggested a combination therapy of tyrosine kinase inhibitor and antifibrotic agent. He also ordered consults from physical therapy and GI.

RJ, 24, was brought to the ER after a car accident. He had blunt trauma to his chest and abdomen, including rib fractures, which were displaced. He also had a right hemothorax. They placed a chest tube in the ER. He was meant to be donating a lung to his girlfriend. When he became unstable, they had to intubate him and rush him into surgery. Because the accident damaged his lungs, they worried he would be unable to be a donor even after he'd recovered. Winston decided to do a wedge resection instead of a lobectomy in order to preserve that option. This was successful and RJ was stable after surgery.
