Jo sits in her car. Suddenly, Levi opens the door and gets in. He's hiding from James. He liked him. They had sex. And then Levi found his wedding album. Jo says that's not a problem. She's pregnant with twins. As he reacts, James knocks on the window. Levi slinks down, but James says he can still see him and asks if they can talk. Then Link knocks on Jo's window. He brought her tea. She pretends she can't hear him. Link says they'll figure it out and proposes some things they can do. Jo starts her car and backs up, leaving James and Link standing there.
Jules gets four coffees from the cart. Simone moves to grab one, but they're all for Jules. She's going to put them in the fridge so she doesn't have to go back to the cart. She's worked two of Mika's shifts this week. Simone has as well. But Mika and Chloe appreciate it. Jules is glad she's recovering well. They pass by a table, where a worker asks if they're registered to vote. Simone, Jules, and Lucas all say yes, while Benson says it's not his thing. The others look at him as he walks away.
Teddy and Owen are in couples counseling. Teddy says lots of couples schedule sex. Marsha says it's perfectly normal to schedule intimacy when you're juggling busy schedules. Owen doesn't want to have to schedule sex with his wife. Marsha asks why it bothers him. Owen feels like Teddy isn't trying to fix anything. They're hemorrhaging and she's reaching for a bandage. Teddy asks him if not wanting to fix their problems the same way he does means she's not trying. Marsha says they have to stop for today, but she wants them to see this through. They should have scheduled sex and see how it feels. It might not be what they think. Owen says he'll see Teddy at 10:00 tomorrow night. Teddy already has to reschedule.
Winston comes to see Monica. She hasn't seen him in a while. He says it's been busy. He has something for her. He hands her an interoffice mail envelope. She pulls out red lingerie. She says he must really be busy because it's not hers. He apologizes and Monica goes to shove it in her pocket as Amelia opens the door and comes in. Amelia sees it hanging out and asks about a consult for Monica. Monica explains that she has a patient coming from Friday Harbor with a head injury. The hospital there is sending over the scans. Amelia offers to come back, but Winston excuses himself and leaves. Amelia tells Monica to have the nurses page her when the scans are in. Monica shoves the lingerie back into the envelope.
Ben tells the interns that Levi is with Beltran today, so he has their assignments. He wants to do a lineup, like at the fire station, but they don't go for it. He says Mika's out. Lucas is with Levi on Beltran's service. Jules is with Ben and Winston. Simone is with Link. Benson says he forgot someone, but Benson has charts that haven't been dictated. He gets to sit at a computer catching up. As they exit, Ben finds Bailey standing nearby and asks if she's lurking. She says his evaluation forms were sent out today. He knows that and knows Winston is evaluating him first. He saw it on her computer. She tells him to be himself.
Lucas says he's never been to Friday Harbor. Levi went once before on a field trip to pick apples. Monica meets them and says they're picking up Ofelia Lopez, a ten year old who fell off a moving tractor. Her vitals are stable, but she has a bad leg injury and a TBI. She needs more care than her local hospital can provide. Levi doesn't mind getting out of the hospital and says it's a nice drive. She corrects that they're going be helicopter. Lucas is excited, but Levi says a ground ambulance would be more cost-efficient. Monica just tells them to update her when they arrive.
Winston, Ben, and Jules round on Darren Riley, who is watching a video of one of his students who is having a hard time learning the steps. Jules presents his case and Ben hands over his morning x-rays. His left lunch is completely whited out. Winston orders a CT with contrast and moves to change Darren's dressing. Ben says he can take care of that, but Winston wants to do it himself. He also wants to help with transportation.
Ben is dictating when he sees Zayne Johnson outside the room, mopping in obvious pain. Zayne dismisses it as sleeping funny and continues. However, soon after, Zayne collapses in the hallway, pulling a shelf on top of himself.
Teddy checks on Aurelio Wheeler, who is eager to go home. Teddy says he can't go quite yet because she saw fluid build up around his heart, which is common after heart surgery. He can't go until they take care of that. He doesn't want to stay because next week is his 40th wedding anniversary. Owen congratulates him. He wants them to see if the fluid resolves on its own. Aurelio continues that he and his wife got engaged when they were nineteen. She wanted a big church wedding, but their parents didn't approve, so they got married at City Hall. Owen says they wanted to get married in a park and ended up getting married at a bar. Every year, the Wheelers recreate their wedding dinner, hot dogs from the cart on the corner. Owen suggests going to Joe's for his and Teddy's anniversary. Teddy says that's the only place they ever go. Teddy wants to run more tests and says they'll take it from there. Aurelio tells Owen to get Teddy flowers on his way home. That always works.
The Friday Harbor doctor thanks them for making the trip. Ofelia has a severe injury to her left leg. She also has head trauma with multiple contusions and lacerations. Her breathing is labored, but they couldn't intubate because their only ventilator is in use and they don't have the resources to manually bag her if they did RSI. Nanette Lopez, Ofelia's mother, says they got her to the hospital as quickly as they could. Jose Lopez, her father, asks if her leg will be all right because she loves to play in the fields. Levi says they'll have a better idea once they examine her. The Friday Harbor doctor says they did a splint a tourniquet and the ischemia time is about 45 minutes. Levi talks to Ofelia, telling her they're going to make her feel better. Levi gives Nanette his number, saying they'll get to Seattle before them. They can't ride with them because there isn't room, so they need to call him when they get there. He promises to take excellent care of her. The Lopezes go to Ofelia, telling her they love her and they'll see her soon. Levi says they'll have to amputate the leg if they don't move quickly, so they need to intubate her for transport as soon as possible.
Bailey sees Benson getting Zayne on a gurney and questions him not working on his charts. He explains what happened. Bailey recognizes Zayne and immediately steps in to say they'll take him down to the ER. He says he's been managing the pain for months just fine. He tries to get them to let him stay, but Benson says he can fight Bailey, but he won't win. Bailey calls Benson to join them as they move to the ER.
A nurse finishes drawing Jo's blood and tells her the doctor will be in soon. Link says they can't fit four car seats into their cars. He wonders if they need to get a mini-van. She reminds him that she's transporting the babies right now. Every horrible pregnancy symptom is doubled. After all of it's done, she'll return to work, delivering other people's babies, with two newborns. Link asks how he can help. Jo can't tell him because all her energy has been sucked out of her. She needs this appointment to be quick because she has a patient who is 29 weeks pregnant with triplets and due to give birth soon. Link says their situation could be worse then. He gets paged for a consult in the ER and she tells him to go. The ultrasound will show the same thing as the last one anyway and he'll smile and say they can handle it and she doesn't need that right now. He leaves.
As Darren gets his CT, Ben asks Winston if he played in the marching band. Winston didn't. Ben didn't either. He looks at Jules. She never did sports. She counts marching band as a sport because they wear uniforms. Darren's CT comes up, showing his lunch is completely liquified from all the necrotic tissue. Ben suggests a chest tube and changing the antibiotics, but Winston says they'll have to remove the whole lung. He goes to get Darren out of the scanner. Once he's gone, Ben asks Jules if Winston always has to do everything himself. Jules says he let her sew a patch graft angioplasty and suggests that maybe it's Ben.
Simone asks Benson about him saying voting isn't his thing. She asks if it's a recent thing. He shakes his head no. She asks him about 2016. He doesn't answer and then Link joins him and Benson presents Zayne's case to him. They're still working on the history and Benson asks if he takes any medication. Zayne lists them off and Bailey realizes he has sickle cell disease. He also knows about the new therapy that cures it, but his insurance won't cover it and he doesn't have the money. Plus if he had the money, he wouldn't want to spend it on treatments. He'd buy a boat and sail around the world. Link says he was in the middle of a patient upstairs and Bailey shows him Zayne's hip x-ray. It's destroyed. Link orders an MRI. Bailey adds an abdominal CT. Zayne tries to object, but Bailey says it won't take long.
The pilot tells Levi and Lucas that they've hit a bit of rain and fog. She's trying to steer them out of it. Levi tries to distract himself by thinking of other things. Lucas notices the many missed calls on Levi's phone and says he's either ghosting someone or someone died. Levi says he's ignoring a mistake. Lucas asks if it's the guy from the hallway and wonders what's wrong with him. Just because it's a bad match doesn't mean it's a mistake. Levi says he's right. It was just a giant failure. The helicopter jolts and Ofelia's alarm beeps. Her blood pressure is up and her heart rate is down. Levi examines her and recognizes the signs of organophosphate poisoning. They start atropine. Ofelia has a seizure and Levi notices that the tourniquet has broken, causing increased bleeding in her leg. Levi holds pressure to stop the bleeding.
Teddy gets Aurelio's results and asks for an ultrasound and a pericardiocentesis kit. Owen still doesn't want to intervene. Teddy says they either do it now or wait until he crashes. She reminds him that he accused her of putting bandages on a hemorrhage. She's trying to face the problem head on and now he doesn't want her to. Her years of experience mean nothing to him. Richard comes in and hears them fighting. He's just there to get some tea. Teddy agrees to give it another hour, but then they'll need more scans.
As they wait for Zayne's scans, Simone asks Benson about local elections and public surveys. He says he sends them straight to spam. Link joins them and Simone tells Zayne to pretend he's on that boat and relax. He says that's not relaxing because he gets seasick. His son is the one who wants to live on a boat and he'd do anything for his son. Zayne's spleen is small and barely functioning. He also has iron deposits in his liver and gallstones. Link says his hip is destroyed. He needs a hip replacement. He won't like it, but he doesn't have a choice.
Monica asks Link if he got Ofelia's films. He says his fellow will have to handle it. Amelia tells Monica she looked at the scans and Ofelia has a minor subdural hematoma. She's not worried, but she'll reevaluate when they land. Monica says their ETA is 25 minutes. Amelia says to page her when they arrive. Monica tells her what she walked in on with her and Winston wasn't what it looked like. Amelia says it's okay.
Lucas can't get the broken tourniquet to hold. Levi says there's an injury to her femoral artery. Levi gets a new tourniquet and they put it on. They'll need to update Monica when they get a signal. Lucas notices the injury to Ofelia's leg is almost beyond repair and thinks she'll need an amputation. Levi has an idea to use IV tubing to make a temporary shunt to buy her the time they need to get her to the hospital. They decide to go for it.
Darren asks if he can go off ECMO if they remove his lunch. Ben says that because of his history of smoking, he'll need to stay on ECMO until he gets a lunch transplant. It could be days or weeks. He keeps thinking of his alto sax, the first instrument he picked up. He fell in love, but he hasn't been able to play in weeks. He asks what if he can never play again or worse, he can't teach. The band is more than just his job. It's his life. Winston knows music and teaching is in his bones and knows this surgery won't change that. He'll just need to adjust.
Zayne is getting dressed to leave when Benson comes in and says Bailey and Link need to talk to him, but he wants to get home to have dinner with his son. Benson tells him about his hip, but he says he'll have to come back later because his son needs attention and Zayne's sister has had him all day. Benson explains what exactly he means about the hip. But the good new is they can replace the hip today and his insurance will cover it. And it'll relieve the pain in his hip. Zayne is bewildered that his insurance company will allow his joints to deteriorate and then they'll pay to fix that instead of preventing it. Benson agrees the system sucks. He's been through it before. But if Zayne cares about himself, he should get the surgery. Simone comes in and says it would be a lot harder for him to take care of his son if he's in constant pain. He agrees to the surgery.
Richard tells Teddy he's about to meet with Balcom Medical. He has a good relationship with Ethan Chen. Teddy tells him Ethan has retired and now the CFO is Erika Desai. She's great, but her husband Nirav is also prominent on the board and he only hears what he wants to hear, which is himself. Teddy starts to rant about that and Richard realizes Teddy is speaking more about herself and Owen than Nirav. He asks if she wants advice or a pep talk. She asks for whatever's less irritating. He tells her they've been through a war together and can handle this.
Winston, Ben, and Jules work on Darren. Ben says his mother wanted him to learn an instrument until he brought home a recorder in fourth grade. Jules and her brother were learning accordion for a few months, but the person teaching them didn't want to be paid with lemons from their yard. As he works, Winston is reluctant to let Ben do anything. Ben says he's on a trial basis, but he did come into the program as a fourth year. He's capable of doing things. Winston ignores him and keeps working. He pulls out Darren's diseased lung.
Bailey checks on Zayne in surgery. It would have been a simpler surgery years ago. It's hard to seek treatment when you have sickle cell. Link tells Benson to be easier on the drill. He tries to take over, but Bailey tells him to let Benson try again. Link still thinks he's going too hard and takes over, only to break the bit. He calls for a new bit and then lets Benson take over as Bailey tells him to go through his business on his own time.
Levi successfully places the tubing as they struggle with the turbulence. He removes the tourniquet and is relieved that it's holding. The pilot says she can't get around the storm and is going to attempt an emergency landing.
Aurelio calls softly for his wife as he crashes. Teddy diagnoses cardiac tamponade, which is why she wanted to do the procedure. She wants to move him to the OR to open his chest and relieve the pressure, while Owen wants to do a needle aspiration bedside. Macie Wheeler, his wife, comes in and then screams when blood starts spurting from his chest.
Teddy has a nurse take Macie out of the room. Suddenly, Aurelio regains consciousness and says he feels much better now. They move to take him to the OR as Macie asks if he's okay. Owen explains that he had fluid building up around his heart, but now that's gone. They'll know more when they get him to the OR, but they need to move now.
Benson asks Simone what she'd do with $3 million. She says she'd fund voter-registration drives. She asks how he thinks the system gets fixed. He says she has to spend it on herself. She says she'd hire an at-home nurse so her granny wouldn't have to live in a facility. Then she'd pay off her loans and start a maternal-health nonprofit. He says that's boring. He would get his own place so he didn't have to live with them and then he'd travel and get a flashy car. But if he had a curable disease, he'd spend every penny on that. Simone finds it odd that he works in a hospital and can't get the treatment he needs paid for. He'd be a great candidate for it. They have everything they need to cure his sickle cell except the money.
Jose asks how they beat the helicopter there. It was a three-hour drive. Monica says when the weather is bad, they will re-route to get around the storm. Jose asks how much longer it'll be before they arrive and asks if Ofelia is still okay. Monica says it doesn't necessarily mean anything, but they've lost contact. Nanette starts crying as Jose goes to call Levi and Lucas. Monica assures them they'll also keep trying until they get through. Nanette says they trusted their team with their daughter and they promised to take care of her. Amelia knows it's terrifying and assures them they're monitoring the situation. They have a whole staff prepared to treat Ofelia as soon as she arrives.
Benson and Simone enter Richard's office. They tell him about Zayne and complain that his insurance company is standing between them and saving him, insulting the insurance company. They hear a sneeze and they peek around the door to see Erika Desai and two other partners from Balcom Medical. Richard sends the interns away and they flee.
Winston says Darren's chest tubes are putting out minimal fluid and he's stable on the ECMO. He tells Ben that Darren knew for a while that he was sick, but he tried to push through it. He decided if he was going to drop, he'd rather do it around people than home alone. Since he showed up at the hospital, Winston's only been able to give him bad news. He's had to take it on by himself. He doesn't have family and his students can't visit. So him wanting to handle Darren's care personally is because he doesn't want him to feel alone, not because he doubts Ben's abilities. He gives Jules and Ben instructions to take over some of Darren's care.
Amelia goes to the helicopter and checks on Monica, who is worried. They should have connected with an ambulance when they made it down, if the made it down. The radio cackles and Amelia uses it to call out, but gets no response. Monica feels guilty because she sent them. She specifically sent Levi in her place. Plus Amelia's nephew. If something happens, she has to call Levi's family and talk to Amelia's family and tell them she put them on the helicopter. Monica says she tends to go to the worst case scenario, which is really hard not to do when they're out there. Amelia says she's experienced a lot and she always goes to the worst case scenario. Sometimes, you have to accept the situation at face value so you can keep moving forward. Monica's phone buzzes. She answers it and looks at Amelia.
Monica and Amelia race outside, followed by Jose and Nanette. They meet the ambulance. Levi is doing chest compressions as they unload Ofelia. Monica takes over as they get her to the ground and start wheeling her inside.
Teddy and Owen scrub out. Owen says Aurelio's heart exploded and now he's totally fine. Teddy starts laughing and they talk about his poor wife, who was covered in his blood. Teddy can still feel the blood in her socks. Teddy says they don't have to schedule sex. Owen says it's their homework and he's always been a straight-A student. Teddy says they'll get through this because they've gotten through a lot worse. Owen asks to take Teddy out on a date, somewhere other than Joe's. She says she'd like that.
Zayne wakes up and they tell him his surgery went well. Richard comes in with Erika. He has good news. There's a possibility he could get the gene therapy he needs to cure him. Erika and her partners at Balcom heard his story and they want to talk to him about applying for a grant they have. He thanks her. Richard tells him to thank Benson and Simone, who stood up for him. Richard says if he gets the go-ahead, they'll need to start soon. The whole process can take more than a year. It'll be at least eight months. Richard talks him through the process. Zayne says he's not sure he can do it because his son has special needs and he needs to be with him. Richard tells him to think about it and maybe he'll figure something out. As they leave the room, Benson tells Simone the system still sucks. Simone reminds him there's something they can do about that.
Levi watches Ofelia sleep for a minute. Then he leaves her room and joins Monica, who is updating Jose and Nanette. She's still in critical condition, but they expect a full recovery with some physical therapy and a likely additional surgery for her leg. They go in to see her and Monica asks if Levi thinks peds is his calling. He tells her a story of being obsessed with helicopters as a kid, only to be scared when he got to ride in one. He felt that same fear on the way to get Ofelia, but when she needed him, he was able to put that aside and only think about her. Even when they were going down, he just wanted to keep her safe. He knows peds is his calling, but he doesn't know how to prove that to everyone else. Monica's never been more certain a resident belongs in peds. She wants to help. He thinks that ship has sailed. That's true for now, but she has friends who got their fellowships after doing research. She has a colleague in Texas who is looking for someone to help with a clinical trial. She asks if he's interested. He says yes, which is good because she already told him Levi would be calling to schedule something. He knows more than he thinks and he needs to trust himself.
Winston asks Ben if he checked on Darren. He's stable for now. The nurses will let them know if anything changes. Winston says he'll handle it if anything does. Not because he doesn't trust Ben, but because he'll only be across the street at Joe's getting dinner. Bailey comes to get Ben. Tuck put the casserole in the oven and Pru is cleaning up her play-doh. Ben says he's going to head to Joe's with Ndugu. Bailey doesn't want to go, but she wasn't invited. Bailey tells them to have a nice time and she'll see Ben at home.
Levi tells Lucas he can go home because Levi can monitor Ofelia. Lucas asks if he's still avoiding that guy. Levi says just because they survived a near-death experience together doesn't mean Lucas can ask him that. Lucas says he's following them. James reaches them and asks Levi if they can talk. Levi says he knows about Ryan. He thought what they had was real and special. James is sorry he didn't say anything but he didn't know when it was the right time to mention his dead husband. Levi didn't realize he was dead. He's been gone nearly four years and James hasn't dated much since then. It's all pretty new to him. He asks if Levi thought he was cheating. Levi tries to deny it, but admits he did. James thinks what they have is real. He's the best thing to happen to James in a long time.
On his way out of the hospital, Benson stops at the voter registration booth and registers.
Monica tells Amelia Ofelia's vascular check looks good. She thanks Amelia for calming her down when she was freaking out. Amelia laughs because she says Monica would see the irony if she knew Amelia better. Monica says the thing with Winston is casual. They're just going through something similar. Amelia tells Monica she doesn't owe her an explanation. They're good.
Jo tells Link the ultrasound showed equal fluid levels and two strong heartbeats. Carina saw nothing to worry about. Link says that's great. Jo apologizes. She was freaked out over one baby and now it's two. She's exhausted with her job. Link reminds her that she locked him out of her car this morning and kicked him out of the ultrasound. She says she was overwhelmed and hormonal. It's a high-risk pregnant. She's scared. He says he's scared, too. They're supposed to be partners and they're falling apart just thinking about having twins. He wonders what'll happen when they're actually born.
Simone comes home to find Lucas cooking. There are lit candles all around the room. He kisses her and tells her he's had a very long day and wanted to end it with her and a real meal. She tells him she's starving.

Zayne, 32, was mopping a floor when he collapsed, pulling a shelf on top of himself. Benson lifted the shelf off him. Zayne said he had several abdominal and hip pain, which had caused his collapse. He said he'd been managing the pain for months. In the ER, he told the doctors he had sickle cell disease. He was aware of the cure, but his insurance wouldn't cover it and he couldn't afford it. After looking at the x-ray of his hip, Link ordered an MRI and Bailey an abdominal CT. The CT showed a spleen that was small and barely functioning, iron deposits in his liver, and gallstones. The MRI also showed he needed a hip replacement due to subchondral collapse. After his hip replacement, he was stable and awake. The head of an insurance company offered him the chance to apply for a grant to get his sickle cell treatment covered, but given the length of time he'd have to be in the hospital, he said he wasn't sure he could accept it.

Owen still opposed. Teddy agreed to give it an hour. Instead of resolving, his condition worsened, leading to cardiac tamponade. Owen wanted to do a needle aspiration in his room, while Teddy wanted to move him to the OR and open him up. He started bleeding profusely, but after a few moments, he woke up and stabilized. They moved him to the OR, where they were able to close his chest.
